Zykadia (ceritinib) is once-daily oral ALK inhibitor for ALK-positive NSCLC. ALK rearrangements ~3–5% of NSCLC. Targeted therapy revolutionized ALK+ NSCLC care.
How Zykadia Works:
ALK fusion proteins cause constitutive kinase activity driving cancer. ALK inhibitors block the kinase activity.
Ceritinib is second-generation ALK inhibitor — broader activity against ALK resistance mutations than crizotinib (first-generation). Alectinib (third-generation, even broader spectrum and better CNS penetration) is generally preferred first-line per ALEX trial.
Form and use:
Zykadia 450 mg once daily with food (reduces GI toxicity). Until disease progression or unacceptable toxicity.
Generic availability:
No generic. ALK inhibitor options: Xalkori (crizotinib), Alecensa (alectinib — generally first-line), Alunbrig (brigatinib), Lorbrena (lorlatinib).
Warnings:
Severe GI toxicity (nausea, vomiting, diarrhea — often dose-limiting; taking with food reduces), hepatotoxicity (LFT monitoring), interstitial lung disease/pneumonitis, QT prolongation, bradycardia, hyperglycemia, pancreatitis.

Diarrhea is one of the most common Tykerb side effects, especially with capecitabine. Start loperamide at the first sign of diarrhea — keep loperamide on hand from the start. Stay hydrated. Severe or persistent diarrhea may require dose reduction or temporary discontinuation. Tell your oncologist about diarrhea that lasts more than 24 hours despite loperamide or is associated with fever, blood, or dizziness.
