Zydelig (idelalisib) is a once-twice-daily oral selective inhibitor of phosphoinositide 3-kinase delta (PI3Kδ) used to treat relapsed chronic lymphocytic leukemia (in combination with rituximab) and relapsed follicular lymphoma and small lymphocytic lymphoma (as monotherapy after at least two prior therapies).


How It Works:

B-cell malignancies (CLL, FL, SLL) depend on signaling pathways downstream of the B-cell receptor (BCR) for cell growth and survival. The PI3Kδ isoform is highly expressed in B-cells and is a critical node in BCR signaling — blocking PI3Kδ disrupts the survival signals that drive these cancers.

Idelalisib selectively inhibits PI3Kδ, blocking activation of downstream effectors (Akt, mTOR) that promote B-cell proliferation, migration, and survival. The result is reduced lymphoma cell growth and induction of apoptosis. Idelalisib is selective for PI3Kδ over the other PI3K isoforms (α, β, γ), which limits some off-target effects but does not eliminate immune-mediated toxicities.

 

Form and Use:
Zydelig is taken as one 150 mg tablet by mouth twice daily, with or without food. For CLL, it is given in combination with rituximab IV. For FL/SLL, it is given as monotherapy after at least two prior systemic therapies. Treatment continues until disease progression or unacceptable toxicity.


Generic Availability:

There is no generic Zydelig in the U.S. as of 2026. To prevent PJP, prophylactic trimethoprim-sulfamethoxazole (Bactrim) is recommended throughout Zydelig therapy and for at least 2–6 months after stopping. CMV reactivation should also be monitored.

Important side effects include diarrhea (sometimes severe and requiring hospitalization), liver enzyme elevations (LFTs are monitored regularly), pneumonitis (any cough or shortness of breath should be evaluated), infections (any fever should be evaluated promptly), and neutropenia. Tell your oncologist promptly about any new GI, respiratory, liver, or infectious symptoms.

If denied, we explore alternatives — switching to a BTK inhibitor (Imbruvica, Calquence, Brukinsa) or venetoclax-based therapy (each with its own PAP), the manufacturer’s copay program for commercially insured patients, or independent foundations such as the Leukemia & Lymphoma Society.
