How Xtandi Works:
Enzalutamide is a second-generation androgen receptor (AR) inhibitor designed to treat prostate cancer, which relies heavily on androgen signaling for growth. It works by binding to the AR with high affinity, blocking androgen binding, preventing the receptor’s nuclear translocation, and inhibiting its interaction with DNA. This comprehensive pathway blockade successfully delays disease progression, reduces symptoms, and improves overall survival. It remains clinically effective even in advanced, castration-resistant stages of the disease where tumors have learned to grow despite low testosterone levels.
Form and use:
Sold under the brand name Xtandi, this medication is administered orally as a 160 mg dose taken once daily, which consists of four 40 mg capsules or tablets. It can be taken either with or without food according to patient preference. Patients are generally required to maintain concurrent androgen deprivation therapy, such as an LHRH agonist/antagonist or surgical castration, throughout the treatment. Additionally, adjustments or avoidance may be necessary if the patient is taking strong CYP3A4 inducers.
Generic availability:
As of 2026, there is no generic version of Xtandi available in the United States. Other alternative androgen-receptor-pathway inhibitors include apalutamide, darolutamide (which may feature less CNS penetration), and abiraterone acetate. Abiraterone acetate works upstream by blocking CYP17 androgen synthesis and currently has multiple generic options available at a substantially lower cost. Because of this, generic abiraterone stands out as often being the most cost-effective AR-pathway therapy available to patients.
Warnings:
Xtandi carries notable warnings for seizures, meaning it should be avoided in patients with a history of seizures or predisposing risk factors. It is also associated with risks of posterior reversible encephalopathy syndrome (PRES), hypersensitivity reactions, ischemic heart disease, and an increased risk of falls and fractures. The drug causes embryo-fetal toxicity, requiring male patients to use condoms during treatment and for three months afterward. Furthermore, it presents significant drug interactions; strong CYP3A4 inducers lower its efficacy, while enzalutamide itself acts as a moderate CYP3A4 inducer that can reduce the effectiveness of many co-administered medications.

The combined approach provides more complete androgen pathway blockade than either alone. The exception is patients who have undergone surgical castration (bilateral orchiectomy).
Yes. Strong CYP3A4 inducers (rifampin, carbamazepine, phenytoin, phenobarbital, St. John’s wort) substantially reduce enzalutamide levels and should be avoided. Enzalutamide itself induces CYP3A4 and CYP2C9 — reducing levels of many co-administered drugs including warfarin (reduces effect), some statins, some antiepileptics, opioids, and others.
