Xiaflex is bacterial collagenase injected directly into Dupuytren’s cords or Peyronie’s plaques to enzymatically digest the abnormal collagen — providing a nonsurgical alternative to fasciectomy (Dupuytren’s) or surgical correction (Peyronie’s).

How Xiaflex Works:

Dupuytren’s contracture involves progressive thickening and shortening of palmar fascia, producing flexion contractures of fingers. Peyronie’s disease involves fibrous plaque formation in tunica albuginea of penis, causing painful curvature.

Xiaflex contains two collagenases that hydrolyze collagen in the affected cord/plaque. Injection followed by 24–72 hour delay then finger extension manipulation (Dupuytren’s) or modeling/stretching (Peyronie’s). Multiple injections per cycle (up to 3 cycles for Peyronie’s, single or multiple cords for Dupuytren’s). Avoids surgical recovery.

Form and use:

Xiaflex injected by trained physician (hand surgeon for Dupuytren’s; urologist for Peyronie’s). Specific protocols for each indication. Patient performs penile modeling exercises post-injection for Peyronie’s.

Generic availability:

No generic. Alternatives — Dupuytren’s: needle aponeurotomy (~$500–$1,500 per finger, can be done in office; higher recurrence than surgery; cheaper than Xiaflex), surgical fasciectomy (more durable but more invasive).
