Xalkori (crizotinib) is a twice-daily oral tyrosine kinase inhibitor of ALK, ROS1, and MET kinases. It was the first FDA-approved ALK inhibitor (2011) for NSCLC patients harboring ALK gene rearrangements — establishing genotype-directed therapy in lung cancer.
How Xalkori Works:
Crizotinib is a targeted inhibitor that blocks the ATP-binding sites of ALK and ROS1 kinases, which are abnormally active in a small percentage of NSCLC patients and drive tumor growth. Blocking these constitutively active fusion kinases triggers rapid tumor regression in many individuals who test positive for these genetic rearrangements. However, most patients eventually develop resistance to the treatment, typically driven by specific mutations like G1202R. Because of this resistance and the need for better central nervous system penetration, newer-generation ALK inhibitors are now generally preferred over crizotinib for standard use.
Form and use:
Marketed under the brand name Xalkori, this medication is available in 200 mg and 250 mg capsules and is taken orally twice a day. The standard adult dosage is 250 mg twice daily, and it can be administered with or without food, while pediatric dosing is calculated based on body surface area. Patients must manage drug interactions carefully, as strong CYP3A inhibitors necessitate a reduction in the crizotinib dose. Conversely, strong CYP3A inducers can significantly lower the drug’s therapeutic levels and must be avoided entirely.
Generic availability:
As of 2026, there is no generic version of Xalkori available in the United States. For first-line treatment of ALK-positive NSCLC, newer-generation options like alectinib, brigatinib, or lorlatinib are preferred due to their broader activity. If treating ROS1-positive NSCLC, alternative therapeutic choices instead include lorlatinib and entrectinib.
Warnings:
Xalkori requires regular liver function tests due to warnings for hepatotoxicity, and it must be permanently discontinued if a patient develops drug-related interstitial lung disease or pneumonitis. It can cause cardiac complications such as QT prolongation and bradycardia, requiring ongoing heart rate and ECG monitoring. Severe visual disturbances, including vision loss, have been reported and necessitate drug discontinuation if they occur. Additionally, the medication carries risks for gastrointestinal perforation, embryo-fetal toxicity, and significant interactions with CYP3A inhibitors and inducers.

Xalkori only works in cancers driven by specific genetic alterations — ALK rearrangements or ROS1 rearrangements (and certain MET alterations in some contexts). Comprehensive genomic profiling of tumor tissue (or sometimes blood) is required to identify the relevant target. Without confirmed ALK or ROS1 alteration, Xalkori would not be effective.
Visual disturbances (light flashes, blurred vision, floaters) are very common, especially early in treatment — most are mild but report them. Other side effects: nausea, diarrhea, edema, fatigue, decreased appetite, transaminitis. Tell your oncologist about new shortness of breath or cough (pneumonitis), severe vision changes, severe abdominal pain, fast or irregular heartbeat, or symptoms of liver problems (yellow skin, dark urine, abdominal pain).
