Vyzulta is once-daily eye drop combining latanoprost (prostaglandin analog) with nitric oxide donor — proposed dual mechanism: uveoscleral outflow (PG) plus trabecular outflow (NO). Marginal benefit over latanoprost alone in trials.
How Vyzulta Works:
Glaucoma involves elevated IOP from impaired aqueous humor outflow. Prostaglandin analogs increase uveoscleral outflow — first-line glaucoma drops.
Vyzulta releases latanoprost acid + nitric oxide. NO theoretically dilates trabecular meshwork, increasing conventional outflow. Net effect: similar or marginally better IOP reduction than latanoprost alone.
Form and use:
Vyzulta 1 drop affected eye(s) once daily in evening.
Generic availability:
No generic Vyzulta. Generic latanoprost very cheap ($5–$15/month). Other generic prostaglandin analogs: bimatoprost (Lumigan generic — once daily; broader eyelash growth side effect), travoprost (Travatan Z generic), tafluprost (Zioptan generic).
Warnings:
Iris pigmentation darkening (irreversible — especially in mixed-color irises), eyelash growth, eyelid skin pigmentation, conjunctival hyperemia, macular edema (rare), iritis/uveitis (rare).
Brand $250–$400/bottle. Generic latanoprost $5–$15/bottle.
Vyzulta = latanoprost + NO. Marginal additional IOP lowering in trials (~1 mmHg). Dramatic cost difference. Generic latanoprost preferred for most patients.
Generic prostaglandin analogs (latanoprost, bimatoprost, travoprost, tafluprost), generic timolol, brimonidine, dorzolamide. Newer brand: Rhopressa (netarsudil), Rocklatan (combination).
