Veltassa (patiromer) is a once-daily oral non-absorbed polymer that binds potassium in the GI tract — particularly in the colon — and eliminates it in stool. It is used for chronic management of hyperkalemia, especially in patients on RAAS inhibitors (ACE inhibitors, ARBs, mineralocorticoid receptor antagonists) where dose reduction or discontinuation would compromise heart failure or CKD management.
How Veltassa Works:
Patiromer is a non-absorbed polymer that treats hyperkalemia—a dangerous condition common in patients with CKD, heart failure, or those using RAAS inhibitors. Working primarily in the colon where potassium levels are higher, it binds potassium in exchange for calcium, allowing the potassium to be eliminated safely through stool. This lowering of serum potassium occurs over a period of seven hours to several days. By managing potassium levels chronically, it enables patients to stay on critical, guideline-directed heart and kidney medications that they would otherwise have to stop.
Form and use:
Veltassa is an oral powder taken once daily, with or without food, by mixing a packet with one-third of a cup of water or selective juices like apple or cranberry. Treatment begins at 8.4 g/day and can be adjusted weekly in 8.4 g steps up to a maximum dose of 25.2 g/day to keep serum potassium within a target range. Crucially, Veltassa must be taken at least 3 hours before or 3 hours after any other oral medications. This separation is vital because the drug can bind to other medications in the GI tract and reduce their absorption.
Generic availability:
As of 2026, there is no generic version of Veltassa available in the United States. Other options for managing chronic hyperkalemia include sodium zirconium cyclosilicate (Lokelma), which is a faster-acting brand-name alternative, as well as dietary changes and loop diuretics. While the older drug sodium polystyrene sulfonate (Kayexalate) is available as a cheaper generic, it is generally avoided for chronic outpatient use due to gastrointestinal side effects and the risk of intestinal necrosis.
Warnings:
Veltassa carries specific warnings regarding the worsening of gastrointestinal motility, meaning it should be avoided by patients with severe constipation, bowel obstructions, or post-surgical motility issues. It can lower absorption of other oral drugs, requiring a strict 3-hour administration buffer. Additionally, it can cause hypomagnesemia, which requires monitoring and potential supplementation of serum magnesium levels. Rare hypersensitivity reactions are also a noted risk.

Veltassa is a charged polymer that can bind other medications in the GI tract — reducing their absorption. Take other oral medications either at least 3 hours BEFORE Veltassa or at least 3 hours AFTER Veltassa. This is especially important for medications with narrow therapeutic windows or where reduced absorption could be clinically significant.
Discuss with your prescriber. Veltassa allows more dietary flexibility regarding potassium-rich foods, but extreme intake may still overwhelm even high-dose Veltassa. Most patients on Veltassa can liberalize their diet somewhat compared with strict low-potassium diet, but should still avoid potassium supplements and very high-potassium foods (salt substitutes containing KCl, large quantities of certain fruits and vegetables) without checking labs first.
