Understanding Tivicay:

Tivicay (dolutegravir, DTG) is a once-daily integrase strand transfer inhibitor (INSTI) used as a component of HIV-1 antiretroviral therapy. It must be combined with other antiretrovirals (NRTI backbone, or rilpivirine in the case of suppressed adults). Dolutegravir has a particularly high genetic barrier to resistance.

How Tivicay Works:

HIV-1 replication requires integrase — a viral enzyme that splices the HIV DNA copy into the host cell’s chromosomes. Without integration, HIV cannot establish persistent infection. INSTIs block this critical step.

Dolutegravir binds HIV integrase at the active site and prevents strand transfer — blocking integration of viral DNA into the host genome. Dolutegravir has a particularly high genetic barrier to resistance, meaning HIV needs to accumulate multiple mutations before becoming resistant — this makes dolutegravir-based regimens more forgiving of occasional missed doses than older HIV drugs.

Form and use:

Tivicay is taken by mouth once daily (or twice daily in patients with INSTI resistance). Available as 10, 25, and 50 mg tablets. Tivicay PD is a 5 mg dispersible tablet for pediatric dosing. Can be taken with or without food. Avoid taking with calcium, iron, magnesium, or aluminum supplements within 2 hours before or 6 hours after.

Generic availability:

There is no generic Tivicay in the U.S. as of 2026. Dolutegravir is also available in fixed-dose combinations: Dovato (DTG/3TC), Triumeq (DTG/ABC/3TC), and Juluca (DTG/RPV). The combination tablets simplify dosing for patients who are also taking other ARVs. HIV guidelines recommend INSTI-based regimens as first-line therapy.

Warnings:

Tivicay carries warnings for hypersensitivity reactions including DRESS (drug reaction with eosinophilia and systemic symptoms), hepatotoxicity (especially with HBV/HCV co-infection), depression/suicidality (rare), immune reconstitution syndrome, embryo-fetal toxicity (initial concern about neural tube defects from preliminary Botswana data, mostly reassured by larger subsequent studies), and weight gain (a class effect with INSTIs). Significant drug interactions occur with polyvalent cations, rifampin, rifabutin, certain antiepileptics, and St. John’s wort.

Strong CYP3A4 inhibitors (ketoconazole, itraconazole, clarithromycin, ritonavir) contraindicated. Moderate inhibitors (fluconazole, diltiazem, erythromycin, verapamil) require dose adjustment. Tell prescriber about all medications.
