Tykerb (lapatinib) is an oral tyrosine kinase inhibitor that targets both HER2 (ErbB2) and EGFR (ErbB1). It is used for advanced or metastatic HER2-positive breast cancer, typically in combination with capecitabine or letrozole.
How Tykerb Works:
Lapatinib is a small-molecule drug that targets HER2-positive breast cancer, an aggressive form of cancer driven by the overexpression of the HER2 receptor. Unlike monoclonal antibodies that target HER2 from outside the cell, lapatinib works from the inside by binding to the ATP-binding site of HER2 and EGFR kinases to block cell growth signaling. Because it can cross the blood-brain barrier, it is uniquely able to target brain metastases. When combined with capecitabine chemotherapy, it provides clinical benefits for patients whose metastatic cancer has progressed on prior therapies.
Form and use:
Lapatinib (Tykerb) is an oral medication taken once daily as a tablet, and it must strictly be taken on an empty stomach—either one hour before or one hour after a meal. Taking this medication with food significantly increases its absorption, which can dangerously raise its toxicity levels. The standard daily dose is 1250 mg (5 tablets) when combined with capecitabine, or 1500 mg (6 tablets) when combined with letrozole.
Generic availability:
A generic version of lapatinib is available in the U.S. at a substantially lower cost than the brand-name Tykerb. It is part of a broader suite of HER2-targeted therapies that include monoclonal antibodies, antibody-drug conjugates, and other oral options like neratinib and tucatinib. The specific choice among these therapies depends on a patient’s prior treatments, disease characteristics, and whether there is brain involvement.
Warnings:
Tykerb carries serious warnings for severe diarrhea, which is often dose-limiting and requires proactive antidiarrheal management. It can also cause severe liver toxicity (hepatotoxicity) and heart issues (cardiotoxicity/LVEF decline), both of which require regular medical monitoring before and during treatment. Additional risks include interstitial lung disease, severe skin reactions, QT prolongation, and embryo-fetal toxicity. Finally, it has significant drug interactions with QT-prolonging drugs as well as CYP3A inhibitors and inducers.

Food significantly increases lapatinib absorption (3–4 fold), which can dangerously raise drug levels and toxicity. Take Tykerb at least 1 hour before or 1 hour after a meal. Avoid grapefruit and grapefruit juice (which also raise lapatinib levels). This dosing requirement is essential.
Tykerb is an oral small-molecule TKI that crosses the blood-brain barrier — making it potentially useful for brain metastases. Trastuzumab (Herceptin) and biosimilars are IV monoclonal antibodies (don’t cross BBB well). Pertuzumab (Perjeta), T-DM1 (Kadcyla), and T-DXd (Enhertu) are also antibody-based. Newer oral options include tucatinib (Tukysa, also crosses BBB and has good brain-metastasis activity) and neratinib (Nerlynx). Treatment choice depends on prior therapies, brain involvement, and the rest of your regimen.
Diarrhea is one of the most common Tykerb side effects, especially with capecitabine. Start loperamide at the first sign of diarrhea — keep loperamide on hand from the start. Stay hydrated. Severe or persistent diarrhea may require dose reduction or temporary discontinuation. Tell your oncologist about diarrhea that lasts more than 24 hours despite loperamide or is associated with fever, blood, or dizziness.
LFTs are checked before starting and every 4–6 weeks during treatment. LVEF (cardiac ejection fraction by echocardiogram or MUGA) is checked at baseline and periodically. ECG may be done to monitor for QT prolongation. Routine CBC and chemistry panels are also done.Tell your oncologist about jaundice, severe fatigue, shortness of breath, palpitations, or persistent diarrhea.
Yes. Lapatinib is metabolized by CYP3A. Strong CYP3A inhibitors (ketoconazole, itraconazole, ritonavir, grapefruit, etc.) substantially raise levels. Strong CYP3A inducers (rifampin, carbamazepine, phenytoin, phenobarbital, St. John’s wort) substantially lower levels and may reduce effectiveness. Tykerb also raises levels of some other CYP3A substrates (digoxin, midazolam). Tell every prescriber and pharmacist about Tykerb.
