Trileptal (oxcarbazepine) is a twice-daily oral antiepileptic that blocks voltage-gated sodium channels — similar to carbamazepine but with improved tolerability (fewer drug interactions, less hyponatremia risk than carbamazepine in many patients, no induction of its own metabolism). Used for partial-onset seizures and off-label for bipolar disorder and trigeminal neuralgia.

How Trileptal Works:

Partial-onset seizures involve excessive neuronal firing. Sodium channel blockers stabilize neuronal membranes and reduce repetitive firing.

Oxcarbazepine is rapidly converted to its active metabolite (10-monohydroxy derivative, MHD) which blocks voltage-gated sodium channels in a use-dependent manner. Compared with carbamazepine, oxcarbazepine has fewer drug interactions (less CYP induction), no auto-induction, and a generally better tolerability profile — though hyponatremia is a recognized side effect.

Form and use:

Trileptal is taken twice daily by mouth. Available as 150/300/600 mg tablets and oral suspension. Adjunctive therapy: start 300 mg BID, increase weekly to target. Renal dose adjustment for CrCl <30. Oxtellar XR is the once-daily extended-release alternative.

Generic availability:

Generic oxcarbazepine is widely available. Other AEDs for partial-onset seizures: lamotrigine (generic), levetiracetam (generic), carbamazepine (generic), valproate (generic), lacosamide (Vimpat, generic), brivaracetam (Briviact), others.

Warnings:
