Trecator (ethionamide) is a second-line antitubercular thioamide derivative used for MDR-TB when first-line agents (isoniazid, rifampin, ethambutol, pyrazinamide) are not effective or cannot be used. Significant GI and hepatic toxicity limits its use.

How Trecator Works:

MDR-TB resists isoniazid AND rifampin — requires second-line regimens. Ethionamide is structurally related to isoniazid; both are activated to active metabolites by EthA enzyme in M. tuberculosis. Active metabolites inhibit mycolic acid synthesis (cell wall component).

Ethionamide is used as part of multidrug MDR-TB regimens. With the advent of BPaL/BPaLM regimens for XDR-TB and treatment-intolerant MDR-TB, the role of ethionamide in modern MDR-TB therapy has decreased — but it remains useful in specific resistance patterns.

Form and use:

Trecator 250 mg tablets taken by mouth, typically 15–20 mg/kg/day in single or divided doses (max 1 g/day). Take with food to reduce GI side effects. Pyridoxine (vitamin B6) supplementation recommended.

Generic availability:

Generic ethionamide available. With BPaL regimen (bedaquiline + pretomanid + linezolid) now preferred for XDR-TB and treatment-intolerant MDR-TB, the role of ethionamide has decreased. Newer/preferred MDR-TB drugs: bedaquiline (Sirturo), pretomanid, linezolid, delamanid, fluoroquinolones (levofloxacin, moxifloxacin), clofazimine.
