Tradjenta is once-daily oral DPP-4 inhibitor — modest A1c reduction (~0.5–0.8%), weight-neutral, low hypoglycemia risk. Linagliptin’s main practical advantage: no renal dose adjustment (unlike sitagliptin, saxagliptin, alogliptin).
How Tradjenta Works:
GLP-1 and GIP incretin hormones augment glucose-stimulated insulin secretion. DPP-4 enzyme rapidly inactivates incretins. DPP-4 inhibition prolongs incretin activity.
Modest glycemic effect. CV-neutral (no benefit or harm in CV outcome trials). SGLT2 inhibitors (Jardiance, Farxiga) and GLP-1 RAs (Ozempic, Mounjaro) have CV/renal benefits beyond glycemia — generally preferred in current ADA/EASD guidelines.
Form and use:
Tradjenta 5 mg once daily by mouth, with or without food.

No generic Tradjenta. Other DPP-4: generic sitagliptin (Januvia generic since 2022 — cheapest), generic alogliptin (Nesina generic), generic saxagliptin (Onglyza generic).
