TOBI is inhaled tobramycin for chronic CF Pseudomonas airway suppression. 28-day on/off cycles to maintain Pseudomonas suppression while limiting resistance pressure.
How TOBI Solution Works:
Pseudomonas aeruginosa chronically colonizes CF airways causing inflammation and lung function decline. Inhaled antibiotics deliver high local concentrations.
Tobramycin aminoglycoside inhibits bacterial protein synthesis. Inhaled formulation: high airway concentration with limited systemic exposure. Cycling reduces resistance pressure while maintaining suppression.
Form and use:
TOBI 300 mg/5 mL via PARI LC PLUS nebulizer BID for 28 days, then 28 days off. ~15 minutes per nebulization. Use bronchodilator before TOBI.
Generic availability:
Generic tobramycin inhalation solution widely available. TOBI Podhaler (dry-powder, brand only). Cayston (aztreonam — different class antibiotic; many CF centers alternate TOBI/Cayston cycles).
Warnings:
Ototoxicity (audiology monitoring), nephrotoxicity (rare with inhaled), bronchospasm (use bronchodilator before), neuromuscular disorder caution.
Brand $5,500–$7,500/cycle. Generic $1,500–$3,000/cycle.
Yes, FDA-approved bioequivalent.
Different inhaled antibiotics. Many centers alternate cycles for year-round Pseudomonas suppression with reduced resistance pressure.
Dry-powder alternative — faster (few minutes vs 15), more portable. Brand only, more bronchospasm.
