Don’t let the high cost of brand specialty inhaled antibiotic therapy disrupt your cystic fibrosis treatment. We help eligible patients access TOBI (tobramycin inhalation solution) for as little as $69.95 per month through the manufacturer’s Patient Assistance Program.
The TOBI Prescription Assistance Program is a manufacturer-sponsored initiative that provides TOBI at little or no medication cost to qualifying patients based on income and insurance status. The program is designed for CF patients with Pseudomonas airway colonization who are uninsured or underinsured.
Navigating the program on your own means dealing with eligibility verification, CF center coordination, prior-authorization documentation, specialty pharmacy logistics, and renewal deadlines.
AffordMyPrescriptions eliminates that burden. For a flat $69.95 per month, our Patient Advocates handle every step of your enrollment.

TOBI (tobramycin inhalation solution) is an inhaled aminoglycoside antibiotic used to chronically suppress Pseudomonas aeruginosa in CF airways. It delivers high local antibiotic concentrations to the airways without significant systemic toxicity. Used in 28-day on / 28-day off cycles to maintain Pseudomonas suppression while limiting resistance pressure.
How TOBI Works:
Most adults and adolescents with cystic fibrosis experience chronic airway colonization by Pseudomonas aeruginosa, which drives progressive lung damage and ongoing inflammation. Tobramycin is an aminoglycoside antibiotic that combats this by binding to the bacterial 30S ribosomal subunit and inhibiting protein synthesis. Administering it via inhalation delivers high antibiotic concentrations directly to airway secretions while avoiding systemic side effects like kidney or ear toxicity. To limit bacterial resistance, the treatment follows a 28-day on/off cycle, and many cystic fibrosis centers alternate it with other inhaled antibiotics for year-round suppression.
Form and use:
The standard liquid formulation of TOBI is administered as a 300 mg dose via a compatible nebulizer twice daily for a 28-day period, followed by 28 days off. Each nebulization session takes roughly 15 minutes, and patients should use a bronchodilator beforehand to maximize lung deposition. Alternatively, the TOBI Podhaler offers a dry-powder option where patients inhale the contents of four capsules twice daily for faster administration.
Generic availability:
Generic versions of the tobramycin inhalation solution are widely available in the United States at a substantially lower cost than brand-name TOBI. However, the dry-powder TOBI Podhaler remains brand-only, as does Cayston, another common inhaled anti-pseudomonal antibiotic. Patients may also utilize compounded inhaled colistin, and many cystic fibrosis centers alternate between generic tobramycin and Cayston during their treatment cycles.
Warnings:
TOBI carries warnings for ototoxicity, which can cause hearing loss, tinnitus, or vertigo, making periodic audiometry monitoring highly recommended. While nephrotoxicity is rare with the inhaled route, kidney function should still be monitored periodically. The medication must be used with caution in patients with neuromuscular disorders like Parkinson’s disease or myasthenia gravis, and it should be avoided during pregnancy due to embryo-fetal toxicity risks. Finally, to prevent bronchospasm, patients should use a bronchodilator before their dose and have their lung function monitored.
Brand TOBI costs approximately $5,500–$7,500 per 28-day cycle. Generic tobramycin inhalation solution is dramatically cheaper. Through AffordMyPrescriptions, qualifying patients receive TOBI at no medication cost — our $69.95 monthly fee covers full advocacy and program management. Most patients find generic tobramycin inhalation solution is the cheaper path.
Yes. The FDA approves generic tobramycin inhalation solution as bioequivalent to brand TOBI. Most CF centers and insurance plans prefer generic.
The original TOBI clinical trials used 28 days on / 28 days off cycling — this regimen reduces continuous antibiotic exposure (limiting resistance pressure) while providing year-round Pseudomonas suppression when alternated with another inhaled antibiotic (typically Cayston aztreonam) during the 28-day off period. Many CF centers do continuous suppression with this alternating regimen — TOBI for 28 days, Cayston for 28 days, repeat.
Both are inhaled anti-pseudomonal antibiotics for chronic CF airway colonization. TOBI (tobramycin, aminoglycoside) — twice daily for 28 days; generic available. Cayston (aztreonam, monobactam) — three times daily for 28 days; brand only (uses Altera Nebulizer). Many CF centers alternate the two for year-round suppression. Cycling between different antibiotic classes also reduces resistance pressure.
TOBI Podhaler is a dry-powder formulation of tobramycin — inhaled via a handheld device instead of nebulization. Faster (a few minutes vs. ~15 min) and more portable than the nebulized form. However, may cause more cough and bronchospasm than the nebulized form for some patients. TOBI Podhaler remains brand only. Discuss with your CF team if convenience is important.
Periodic audiometry (hearing test) — aminoglycosides can cause cumulative ototoxicity; check at baseline and approximately annually. Renal function (creatinine) periodically — though systemic absorption from inhalation is low. Lung function (FEV1) regularly. Sputum culture/sensitivity periodically to monitor for resistance development.
If denied, we explore alternatives — switching to generic tobramycin inhalation solution (much cheaper), Cayston (aztreonam inhalation), the manufacturer’s copay program for commercially insured patients, the Cystic Fibrosis Foundation Compass program (for CF patients), or independent foundations such as the CF Foundation, PAN Foundation, or HealthWell Foundation. If we cannot find a path, you won’t be charged our service fee.
If you are struggling with the high cost of TOBI, our team may be able to help you access assistance programs — or guide you to generic tobramycin inhalation solution. Check your eligibility today.
