Tasigna (nilotinib) is a twice-daily oral second-generation BCR-ABL tyrosine kinase inhibitor (TKI) used for Philadelphia chromosome-positive chronic myeloid leukemia (CML). It is more potent than imatinib and achieves faster, deeper molecular responses for many patients.
How Tasigna Works:
Chronic Myeloid Leukemia (CML) is driven by the BCR-ABL fusion gene, which creates an active tyrosine kinase that causes uncontrolled cell growth. Nilotinib is a second-generation tyrosine kinase inhibitor designed to be more potent and selective than older drugs like imatinib, achieving deeper and faster molecular responses. It binds directly to the BCR-ABL protein to shut down its harmful kinase activity. Patients who achieve a sustained, deep molecular response over several years may eventually become candidates for treatment-free remission under close medical monitoring.
Form and use:
Tasigna is an oral medication that must be taken twice daily on an empty stomach, specifically at least 2 hours after eating and 1 hour before the next meal. Taking it with food is dangerous because it significantly increases drug absorption to unsafe levels. The standard adult dose is 300 mg twice daily for newly diagnosed chronic phase CML, which increases to 400 mg twice daily for resistant or intolerant cases. Pediatric dosages differ from adult versions as they are calculated based on the patient’s body surface area.
Generic availability:
Generic nilotinib is currently available in the United States, offering a substantially lower-cost alternative to the brand-name Tasigna. Beyond nilotinib, there are several other tyrosine kinase inhibitors used to treat CML, including generic options like imatinib and dasatinib. Other specialized treatments in this drug class include bosutinib, ponatinib (which specifically targets the T315I mutation), and a novel STAMP inhibitor called asciminib.
Warnings:
Tasigna carries a boxed warning for QT prolongation and sudden death, making baseline and ongoing ECG monitoring mandatory alongside regular electrolyte corrections. Food must be strictly avoided during dosing hours because it spikes drug absorption and significantly increases these dangerous cardiac risks. The medication also requires close monitoring for myelosuppression, hepatotoxicity, pancreatitis, hyperglycemia, and tumor lysis syndrome at the start of therapy. Finally, it can cause severe arterial occlusive disease, ischemic heart disease, and embryo-fetal toxicity.

Food significantly increases nilotinib absorption — taking Tasigna with food can dangerously raise drug levels and increase the risk of QT prolongation and sudden death (the boxed warning concern). Take Tasigna at least 2 hours after the previous meal and 1 hour before the next meal. Avoid grapefruit and grapefruit juice (which also raise nilotinib levels). This dosing requirement is essential — don’t skip it.
Both are BCR-ABL TKIs for CML. Tasigna (nilotinib) is a second-generation TKI — more potent, deeper molecular responses faster than imatinib (Gleevec). Tasigna is sometimes preferred for selected high-risk newly diagnosed patients who would benefit from deeper, faster responses. Trade-offs: Tasigna requires twice-daily dosing on an empty stomach and has QT prolongation and vascular event risks; Gleevec is once daily with food and has a longer safety track record. Many CML patients do excellently on either drug.
Some CML patients who have been on Tasigna for many years and have achieved a sustained deep molecular response (MR4.5) for at least 1–2 years may be candidates for stopping nilotinib under careful monitoring — a ‘treatment-free remission’ (TFR) attempt. About 50% of carefully selected patients maintain molecular remission off TKI; the others require restarting therapy. TFR should only be attempted under expert oncology guidance with frequent molecular monitoring.
Important side effects include cardiac/vascular events (chest pain, leg pain, sudden onset of severe headache, vision changes, signs of stroke — call 911), arrhythmias from QT prolongation, low blood counts (signs of infection, bleeding/bruising), elevated blood sugar (diabetes monitoring), elevated lipase/amylase (signs of pancreatitis), rash, headache. Tell your oncologist promptly about any new or worsening symptoms.
Yes — many. Nilotinib is metabolized by CYP3A and is itself a CYP3A inhibitor. Strong CYP3A inhibitors and inducers, antiarrhythmics, other QT-prolonging drugs (some antibiotics, antifungals, antidepressants, antipsychotics, antiemetics), proton pump inhibitors, antacids, statins, and warfarin can all interact significantly.
