Synarel is intranasal GnRH agonist. Continuous GnRH stimulation paradoxically suppresses pituitary LH/FSH after initial ‘flare,’ leading to hypogonadism (medical menopause for women, halting puberty in CPP).
How Synarel Works:
GnRH normally pulses; continuous receptor stimulation desensitizes/downregulates pituitary GnRH receptors. After initial 1–2 week flare of LH/FSH release, gonadotropins fall to castrate levels.
For endometriosis: estrogen suppression reduces ectopic endometrial growth and symptoms. For CPP: suppresses precocious gonadal activation. Maximum 6 months of GnRH agonist due to bone loss; add-back therapy (low-dose hormones) used for longer courses.
Form and use:
Synarel nasal spray BID (one spray each nostril morning and evening) for endometriosis. CPP dose varies.
Generic availability:
Synarel brand only currently. Generic leuprolide IM injection (Lupron Depot generic) — most cost-effective GnRH agonist. Goserelin (Zoladex), triptorelin (Trelstar) other options. Elagolix (Orilissa) oral GnRH antagonist alternative for endometriosis.
Warnings:
Bone loss with prolonged use (>6 months without add-back), hypoestrogenic symptoms (hot flashes, vaginal dryness, mood changes, libido), depression, possible cardiovascular effects, transient initial flare (worsening symptoms in first 1–2 weeks), seizures (rare), pituitary apoplexy (very rare in unrecognized pituitary adenoma).

Synarel BID intranasal (more frequent). Leuprolide monthly or every-3-month injection. Generic leuprolide much cheaper. Nasal spray useful for patients preferring no injections.

Low-dose estrogen/progestin (norethindrone) added with GnRH agonist for >6 months to prevent bone loss and reduce hypoestrogenic symptoms.
DEXA scans recommended for prolonged courses (>6 months). Calcium/vitamin D supplementation, weight-bearing exercise.
