Samsca (tolvaptan) is a selective vasopressin V2 receptor antagonist used to treat clinically significant hyponatremia in hypervolemic or euvolemic conditions. By blocking V2 receptors in the kidney, Samsca causes aquaresis — excretion of free water without sodium — raising serum sodium back toward normal.
How Samsca Works:
Hypervolemic and euvolemic hyponatremia are driven by excessive water retention caused by elevated antidiuretic hormone activity. Tolvaptan treats this by selectively blocking V2 vasopressin receptors in the kidney collecting ducts, which prevents the insertion of aquaporin water channels. As a result, the kidneys become impermeable to water and excrete dilute urine, a process called aquaresis. This promotes free water excretion without losing sodium, effectively raising serum sodium levels.
Form and use:
Samsca is initiated in a hospital setting at 15 mg orally once daily, with serum sodium measured every 2 to 6 hours initially. The dose can be increased to 30 mg or 60 mg as needed, ensuring the maximum sodium correction stays below 12 mEq/L in any 24-hour period. After safe in-hospital initiation, patients can continue treatment as outpatients. However, therapy must be limited to a maximum duration of 30 days due to hepatotoxicity concerns, and any re-initiation requires a new hospital admission.
Generic availability:
Generic tolvaptan is currently available in the U.S. at a substantially lower cost than the brand-name Samsca. Other vasopressin receptor antagonists include conivaptan, which is restricted to intravenous hospital use, and the older generic drug demeclocycline. For managing chronic SIADH, lifestyle modifications like fluid restriction, salt tablets, and loop diuretics are often attempted first. Notably, the brand Jynarque also contains tolvaptan but features different dosing, is indicated for ADPKD, and has its own separate REMS program.
Warnings:
Samsca carries a Boxed Warning for severe, potentially fatal hepatotoxicity, requiring treatment to be limited to 30 days or less and avoiding use in patients with cirrhosis. It also carries a Boxed Warning for rapid sodium correction exceeding 12 mEq/L in 24 hours, which can cause osmotic demyelination syndrome and necessitates in-hospital monitoring. Additional warnings include risks of dehydration, hypotension, and volume depletion when combined with concurrent diuretics. Finally, patients must be monitored for hyperkalemia and significant drug interactions with CYP3A inhibitors and inducers.

Samsca causes significant free water excretion and can correct sodium too quickly — overly rapid correction risks osmotic demyelination syndrome, a severe and sometimes irreversible neurologic disease. In-hospital initiation allows frequent sodium monitoring (every 2–6 hours initially) so the dose can be adjusted to keep correction safe (<12 mEq/L per 24 hours). Outpatient continuation is fine once safe in-hospital titration is complete.
Samsca’s labeling limits total exposure to ≤30 days due to hepatotoxicity risk. For longer therapy, fluid restriction, salt tablets, loop diuretics, urea, or addressing the underlying cause (treating heart failure, evaluating SIADH cause) are alternatives. If the underlying condition causing hyponatremia is treatable (e.g., adjusting heart failure regimen), that is preferred over chronic Samsca.
Tolvaptan can cause severe, sometimes fatal liver injury — the boxed warning. LFTs are monitored at baseline and during therapy. Significant transaminase elevations require stopping Samsca. Patients with underlying liver disease (especially cirrhosis) should not receive Samsca. The 30-day duration limit also reflects hepatotoxicity concern.
Samsca causes ‘aquaresis’ — increased excretion of free water. You’ll urinate large volumes of dilute urine. Drink water freely whenever thirsty to prevent dehydration. This is the intended effect of the medication. Manage by anticipating frequent urination (plan bathroom access, avoid drinking immediately before sleep).
Yes. Strong CYP3A inhibitors (ketoconazole, clarithromycin, ritonavir) substantially raise tolvaptan levels — contraindicated. Strong CYP3A inducers (rifampin, carbamazepine, phenytoin, St. John’s wort) substantially lower levels. Other diuretics or RAAS inhibitors may potentiate volume depletion. Tell every prescriber and pharmacist about Samsca.
