Pneumovax 23 (PPSV23) is a 23-valent pneumococcal polysaccharide vaccine — historical adult pneumococcal vaccine. Covers 23 pneumococcal serotypes but with less durable immunity than conjugate vaccines. Given after PCV15 (Vaxneuvance) in current ACIP recommendation; alternatively single-dose PCV20 or PCV21 (Capvaxive) can be used.
How It Works:
Streptococcus pneumoniae (pneumococcus) causes pneumonia, meningitis, sepsis, and other invasive disease. Adults 65+ and adults with certain medical conditions have highest disease risk. Historical adult pneumococcal vaccination used PPSV23 (Pneumovax 23) alone or in sequence with conjugate vaccines. Pneumovax 23 (PPSV23) is a polysaccharide vaccine — direct pneumococcal capsular polysaccharides for 23 serotypes elicit antibody response. Broader serotype coverage than early conjugate vaccines but T-cell-independent response with less durable immunity and memory. Modern conjugate vaccines (PCV13, PCV15, PCV20, PCV21) provide T-cell-dependent responses with durable protection. Current ACIP 2024 recommendation for adults 50+: one dose of PCV20 or PCV21 (Capvaxive), OR PCV15 (Vaxneuvance) followed 1+ year later by PPSV23 (Pneumovax 23). Additional considerations for immunocompromised or high-risk conditions.
Form and Use:
Pneumovax 23 0.5 mL IM or SC injection in deltoid. Timing depends on ACIP recommendation and prior vaccination history. Given at pharmacy, clinic, or occupational health.
Generic Availability:
No generic Pneumovax 23. Modern pneumococcal vaccine landscape (all brand): PCV15 (Vaxneuvance), PCV20 (Prevnar 20), PCV21 (Capvaxive — 21-valent adult-focused serotypes). ACIP 2024: adults 50+ receive one dose of PCV20 or PCV21, OR Vaxneuvance (PCV15) followed by Pneumovax 23. Under ACA and IRA Medicare Part D, ACIP-recommended vaccines covered without cost-sharing. Merck Vaccine Patient Assistance Program for uninsured. State/local health departments. CDC and ACIP resources.
Warnings:
Anaphylaxis and severe allergic reactions (rare — do not administer to persons with severe allergic reaction to prior pneumococcal vaccine or component), embryo-fetal considerations (safety in pregnancy not fully established — weigh risk-benefit), immunocompromised (may have reduced immune response). Other: injection site pain, redness, swelling; fatigue, headache, myalgia, low-grade fever. Not a live vaccine — safe for immunocompromised from a live-vaccine standpoint. Moderately potent local reactions in some — particularly with revaccination close to prior dose.
$100–$150 per dose at pharmacies. Under ACA, ACIP-recommended vaccines covered without cost-sharing for privately insured. IRA — Medicare Part D covers adult vaccines at $0 (2023). Verify with insurance.
Pneumovax 23 is a polysaccharide vaccine (PPSV23) — direct polysaccharides for 23 serotypes; broader serotype coverage historically but less durable immunity. PCV20 and PCV21 are conjugate vaccines — polysaccharides linked to carrier protein for T-cell-dependent durable response. Current ACIP 2024 recommendation for adults 50+: one dose of PCV20 or PCV21 (Capvaxive), OR PCV15 followed 1+ year later by PPSV23 (Pneumovax 23). Single-dose PCV20/PCV21 simpler regimen.
In current ACIP guidance (2024), adults 50+ receiving PCV15 (Vaxneuvance) should receive PPSV23 (Pneumovax 23) 1+ year later. Alternative: single dose of PCV20 or PCV21 (Capvaxive) — no PPSV23 needed. Adults with certain immunocompromising conditions may need additional PPSV23 dose per ACIP. Discuss with primary care.
Generally no. Current ACIP 2024 recommendation: PCV20 or PCV21 (Capvaxive) alone is sufficient for most adults 50+ — no additional PPSV23 needed. If you received PCV15 (Vaxneuvance) instead, PPSV23 (Pneumovax 23) should follow. Discuss with primary care based on your specific vaccination history.
IM or SC injection in deltoid. Given at pharmacy, clinic, or provider office. Under ACA and IRA Medicare Part D, most adults have $0 cost-share for ACIP-recommended vaccines.
