Unlike rilpivirine, doravirine does not require a substantial meal for absorption and does not interact with acid-reducing medications — making it more convenient than older NNRTIs in many situations.
How Pifeltro Works:
HIV is a retrovirus that relies on the reverse transcriptase enzyme to replicate and integrate its genetic material into a host’s genome. Doravirine is an NNRTI that binds to an allosteric site on this enzyme, distorting it so it can no longer convert viral RNA into DNA. Unlike older medications in its class, doravirine remains effective against HIV-1 strains carrying common NNRTI resistance mutations like K103N, Y181C, and G190A. To effectively prevent viral replication and resistance, it must be used as part of a combination therapy regimen.
Form and use:
Pifeltro is administered as a single 100 mg tablet taken orally once daily, and it can be taken with or without food. It cannot be used alone and must be combined with two other antiretroviral medications, which are typically NRTIs like generic FTC or 3TC paired with tenofovir. Unlike some other treatments like rilpivirine, Pifeltro does not require a meal for proper absorption. Additionally, its effectiveness is not impacted by concurrent use with acid-reducing medications.
Generic availability:
As of 2026, there is no generic version of Pifeltro available in the U.S. However, doravirine is available as a component of Delstrigo, which combines it with lamivudine and tenofovir disoproxil fumarate into a complete, single-tablet regimen. Other NNRTI alternatives include efavirenz and rilpivirine, the latter of which is found in combination products like Complera and Cabenuva. Despite these options, modern HIV guidelines generally prefer integrase-inhibitor-based regimens for patients who are treatment-naïve.
Warnings:
Pifeltro carries warnings for hepatotoxicity, particularly in patients who have an HBV or HCV co-infection, as well as risks for immune reconstitution syndrome. Patients should also be monitored for rare neuropsychiatric events, depression, suicidality, and changes in body fat redistribution. It interacts significantly with strong CYP3A inducers like rifampin, St. John’s wort, and certain anticonvulsants, which drastically lower doravirine levels and should be avoided. If taken alongside rifabutin, the Pifeltro dose must be increased to twice daily.

NNRTI-based regimens (including Pifeltro and Delstrigo) are still effective and useful — particularly in patients with INSTI intolerance or specific situations — but are no longer first-line for most treatment-naïve patients.
Yes. Unlike rilpivirine, doravirine does NOT require stomach acid for absorption, so PPIs (omeprazole, esomeprazole, etc.), H2 blockers, and antacids do not significantly interact with Pifeltro. This is a practical advantage.
Yes. Medicare Part D beneficiaries can typically qualify, especially if you face specialty-tier copays. State ADAP programs may also coordinate with Medicare for HIV-treatment patients.
