Understanding Ongentys:

Ongentys is a once-daily peripheral catechol-O-methyltransferase (COMT) inhibitor used as adjunctive therapy to levodopa/carbidopa in Parkinson’s disease patients experiencing motor fluctuations (‘off’ episodes). The convenient once-daily dosing differentiates it from older entacapone (3–8 times daily) and tolcapone (3 times daily).

How Ongentys Works:

Levodopa is the gold-standard PD therapy — converted to dopamine in the brain. Levodopa is metabolized peripherally by COMT (to 3-O-methyldopa) and AADC (to dopamine in the periphery, which doesn’t cross BBB). Adding carbidopa blocks AADC; adding a COMT inhibitor (entacapone, tolcapone, opicapone) blocks COMT — increasing levodopa’s bioavailability and prolonging effect, reducing ‘off’ time.

Opicapone is a third-generation peripheral COMT inhibitor with very long-lasting COMT blockade — allowing once-daily dosing (vs. with-every-dose dosing for entacapone). This improves adherence and reduces pill burden.

Form and use:

Ongentys 50 mg capsule taken once daily at bedtime, at least 1 hour before or after consuming any food or other medications (food and other drugs reduce absorption).

Generic availability:

There is no generic Ongentys. Other COMT inhibitors: entacapone (Comtan, generic — taken with each levodopa dose, 3–8 times daily; very cheap generic; combination Stalevo includes levodopa/carbidopa/entacapone in one pill, also generic), tolcapone (Tasmar, generic — three times daily with hepatotoxicity warning requiring LFT monitoring).

Warnings:

Ongentys carries warnings for dyskinesias (may worsen — may need to reduce levodopa dose when adding), hypotension/syncope, hallucinations and psychotic-like behavior (more common in elderly with PD), impulse control disorders (class effect of dopaminergic therapies), withdrawal syndrome with abrupt discontinuation, and rhabdomyolysis (rare).

Yes — adding a COMT inhibitor increases levodopa bioavailability. Your neurologist will likely reduce your levodopa dose when starting Ongentys to prevent excessive dyskinesias. Don’t change levodopa dose without medical guidance.Limited data exist on combining oral gepants with anti-CGRP monoclonal antibodies. Theoretically both target the CGRP pathway, so significant additive benefit for prevention may be limited — but the safety of using a gepant for acute treatment while on a preventive monoclonal antibody appears reasonable. Discuss with your neurologist.

Avoid non-selective MAOIs (older MAOIs); selective MAO-B inhibitors (rasagiline, selegiline) generally OK. Avoid concomitant catecholamine-based drugs at high doses (epinephrine, norepinephrine, isoproterenol, dobutamine, dopamine, methyldopa).
