Nurtec ODT (rimegepant) is an oral CGRP receptor antagonist (gepant) used both for the acute treatment of migraine attacks and for migraine prevention in adults with episodic migraine. It is unique in being approved for both indications, allowing one medication to serve both purposes for many patients.
How Nurtec ODT Works:
Migraine attacks are driven by the trigeminovascular system, where nerve fibers release CGRP to cause vasodilation, inflammation, and pain. Rimegepant works by blocking the CGRP receptor to stop this process and relieve migraine pain and associated symptoms. Unlike traditional triptans, gepants do not cause vasoconstriction, making them a safe alternative for patients with cardiovascular disease. The medication can be used both as an acute treatment and as an every-other-day preventive therapy to reduce migraine frequency.
Form and use:
Nurtec ODT is available exclusively as an orally disintegrating tablet that dissolves on or under the tongue and can be taken with or without food. For acute migraine treatment, the dose is 75 mg orally as needed, with a strict limit of one dose per 24 hours and a maximum of 18 doses per month. For migraine prevention, the required dose is 75 mg taken orally every other day.
Generic availability:
As of 2026, there is no generic version of Nurtec ODT available in the U.S. Other gepant alternatives include oral ubrogepant for acute use, once-daily oral atogepant for prevention, and intranasal zavegepant for acute treatment. Prevention options also include anti-CGRP monoclonal antibodies like Aimovig, Ajovy, Emgality, and Vyepti. For older, widely used alternatives, generic triptans remain common for acute attacks, while generic beta-blockers, topiramate, and amitriptyline are used for prevention.
Warnings:
Nurtec ODT is generally well-tolerated, with nausea and abdominal pain being the most common adverse reactions. Hypersensitivity reactions, such as dyspnea and rash, have been reported by some patients. Drug levels can be significantly impacted by metabolic enzymes, so concurrent use with strong CYP3A inhibitors or strong to moderate CYP3A inducers should be avoided. Additionally, this medication is not recommended and should be avoided in patients with severe hepatic impairment or end-stage renal disease.

Triptans (sumatriptan, rizatriptan, etc.) work by causing vasoconstriction of cranial blood vessels and reducing CGRP release. They are effective but contraindicated in patients with cardiovascular disease, stroke history, or uncontrolled hypertension. Gepants like Nurtec ODT block the CGRP receptor without causing vasoconstriction — making them appropriate for patients who cannot take triptans. Some patients also prefer gepants for tolerability.
Both are oral gepants for acute migraine. Nurtec ODT (rimegepant) is also approved for prevention — every-other-day dosing. Ubrelvy (ubrogepant) is for acute use only. Both have similar acute efficacy. The choice depends on whether you want one medication for both acute and prevention use, and on insurance coverage.
Limited data exist on combining oral gepants with anti-CGRP monoclonal antibodies. Theoretically both target the CGRP pathway, so significant additive benefit for prevention may be limited — but the safety of using a gepant for acute treatment while on a preventive monoclonal antibody appears reasonable.
