Nuedexta is the only FDA-approved treatment for pseudobulbar affect (PBA) — uncontrollable laughing or crying episodes that occur in some neurologic conditions. Combines dextromethorphan (NMDA receptor antagonist, sigma-1 agonist, serotonin/norepinephrine reuptake inhibitor) with low-dose quinidine (a CYP2D6 inhibitor that prevents dextromethorphan breakdown — allowing therapeutic exposure).
How Nuedexta Works:
PBA involves disrupted neural circuits connecting cortical regulation of emotional expression. The exact mechanism by which dextromethorphan modulates these circuits is unclear, but multiple neurotransmitter effects (NMDA, sigma-1, serotonin, norepinephrine) likely contribute.
Dextromethorphan alone is rapidly metabolized by CYP2D6, so oral dosing wouldn’t achieve effective levels. Quinidine at very low dose inhibits CYP2D6 — allowing dextromethorphan to reach therapeutic concentrations. The combination reduces frequency of PBA episodes.
Form and use:
Nuedexta capsules taken by mouth once daily for the first 7 days, then twice daily (every 12 hours). Take with or without food.
Generic availability:
Generic dextromethorphan/quinidine combination is available at lower cost. Off-label alternatives for PBA: generic SSRIs (sertraline, citalopram), generic TCAs (amitriptyline, nortriptyline) — multiple meta-analyses show benefit, all very cheap. Many neurologists try SSRI first before Nuedexta.
Warnings:
Nuedexta carries warnings for QT prolongation (especially with other QT-prolonging drugs; quinidine contributes), drug interactions (quinidine inhibits CYP2D6 — affecting many drugs metabolized by CYP2D6 including codeine, tamoxifen, certain antidepressants/antipsychotics), hepatotoxicity (rare), thrombocytopenia (rare; quinidine-related), serotonin syndrome (especially with serotonergic drugs), falls/dizziness, and contraindications including patients on MAOIs.

Often yes. SSRIs (sertraline, citalopram — generic, very cheap) have evidence for PBA and are typically tried first. Nuedexta is the only FDA-approved PBA treatment, but generic SSRIs are dramatically cheaper and often effective.
Quinidine inhibits CYP2D6 — affects metabolism of many drugs: codeine (converted to morphine by CYP2D6 — pain control may be reduced), tamoxifen, many antidepressants (paroxetine, fluoxetine, etc.), many antipsychotics. Tell every prescriber about Nuedexta.
Quinidine at very low dose (10 mg) doesn’t have its own antiarrhythmic effect, but inhibits CYP2D6 — preventing dextromethorphan from being rapidly metabolized. Without quinidine, oral dextromethorphan wouldn’t reach therapeutic levels.
