Noxafil (posaconazole) is an oral and IV extended-spectrum triazole antifungal used for prevention and treatment of invasive fungal infections — particularly Aspergillus species and Candida — in immunocompromised patients (stem cell transplant, hematologic malignancy with prolonged neutropenia, solid organ transplant, advanced HIV). It also has activity against some less common molds.
How Lovenox Works:
Fungal infections pose serious risks to immunocompromised patients, making prophylactic therapy essential to reduce invasive infections during high-risk periods like neutropenia. Posaconazole works by blocking ergosterol synthesis in the fungal cell membrane, rendering the membrane dysfunctional and causing the fungi to die. It features broad-spectrum activity against Aspergillus, most Candida species, and various other molds and dimorphic fungi. The delayed-release tablet formulation offers more reliable absorption than the older oral suspension.
Form and use:
Noxafil is available as delayed-release (DR) tablets, an oral suspension, and an intravenous (IV) formulation. The DR tablets and IV version are typically dosed at 300 mg twice daily on day one, followed by 300 mg once daily for prophylaxis. The oral suspension requires a dose of 200 mg three times daily and must be taken with a high-fat meal or nutritional supplement due to food-dependent absorption. Because of its more reliable absorption, the DR tablet is generally preferred over the suspension for most adult patients.
Generic availability:
Generic posaconazole delayed-release tablets are currently available in the U.S. at a substantially lower cost than the brand-name Noxafil. Other extended-spectrum triazole options include voriconazole for invasive aspergillosis, isavuconazole for mucormycosis, and the older itraconazole. Fluconazole is a very cheap generic alternative that covers many Candida species but fails to target Aspergillus or most molds. Additionally, IV echinocandins like caspofungin, micafungin, and anidulafungin are available to cover Candida and some Aspergillus.
Warnings:
Noxafil requires regular monitoring due to warnings regarding hepatotoxicity, QT prolongation, arrhythmias like torsades de pointes, and electrolyte abnormalities. Rare mineralocorticoid effects, such as pseudo-hyperaldosteronism causing hypertension and hypokalemia, can also occur with prolonged use or high doses. Furthermore, it acts as a potent CYP3A4 inhibitor, leading to significant and sometimes contraindicated drug interactions with medications like statins, vincristine, and calcineurin inhibitors.

Prophylaxis duration depends on indication: throughout neutropenia in stem cell transplant or AML/MDS chemotherapy (often weeks to months), until adequate immune recovery in advanced HIV (typically months), throughout GVHD treatment in transplant patients (often months).
