Miebo is unique — semifluorinated alkane that spreads on tear film surface, reduces evaporation. First FDA-approved drug specifically targeting tear film evaporation. For evaporative dry eye (most common form, associated with meibomian gland dysfunction).
How Miebo Works:
Most dry eye is evaporative — meibomian gland dysfunction reduces lipid layer, tears evaporate too fast.
Perfluorohexyloctane forms monolayer at tear surface — reduces evaporation. Different mechanism from cyclosporine/lifitegrast (anti-inflammatory) or tears (volume replacement).
Form and use:
Miebo 1 drop in each eye 4 times daily. Wait 5 min before/after other eye drops.
Generic availability:
No generic Miebo (novel 2023 approval). Other Rx dry eye: cyclosporine (Restasis — generic available; Cequa, Vevye), lifitegrast (Xiidra), varenicline nasal spray (Tyrvaya), loteprednol (Eysuvis — short-term flare). OTC artificial tears extensive options.
Warnings:
Blurred vision, eye redness, generally well-tolerated. Avoid contact lens wear immediately around dosing.
$700–$900/bottle. OTC tears $15. Generic cyclosporine $50–$100.
Miebo targets evaporation — root cause of most dry eye. Other drops anti-inflammatory (Restasis, Xiidra) or volume replacement (tears). Some patients benefit from combination.
Meibomian gland dysfunction — blocked oil glands in eyelid margin. Warm compresses, lid hygiene, omega-3s, sometimes thermal procedures help.
OTC tears, warm compresses, lid hygiene, generic cyclosporine, Tyrvaya samples.
