Trifluridine is a cytotoxic thymidine analog that incorporates into DNA during replication, causing damage and cell death in rapidly dividing cancer cells. Because trifluridine alone is quickly broken down by an enzyme called thymidine phosphorylase in the liver and gastrointestinal tract, it is paired with tipiracil. Tipiracil acts as an inhibitor that blocks this breakdown, ensuring trifluridine safely reaches systemic circulation at effective therapeutic levels. This oral combination provides a modest overall survival benefit in advanced colorectal cancer, which can be further improved when paired with bevacizumab.
Form and use:
Lonsurf is an oral medication taken twice daily and must be consumed within one hour of finishing morning and evening meals. The standard dosage is based on the body’s surface area at 35 mg/m² per dose, rounded to the nearest 5 mg, up to a maximum of 80 mg per dose. It is administered on a strict schedule within a 28-day cycle: patients take the medication on days 1 through 5 and days 8 through 12, followed by 14 days off. Because of these specific timing requirements, it must always be taken with food.
Generic availability:
As of 2026, there is no generic version of Lonsurf available in the United States. Depending on prior therapies and biomarker profiles, other later-line treatment choices for metastatic colorectal cancer include regorafenib, which does have a generic version, and fruquintinib. For metastatic gastric cancer, alternative options include ramucirumab, nivolumab, and trastuzumab deruxtecan. Healthcare providers base the final choice of these therapies on a patient’s treatment history, specific biomarkers, and overall tolerability.
Warnings:
Lonsurf carries significant warnings for severe and potentially life-threatening myelosuppression, making complete blood count monitoring mandatory before and during every treatment cycle. This suppression commonly leads to side effects such as neutropenia, thrombocytopenia, and anemia, which may necessitate delays or reductions in the medication’s dose. The drug also carries a warning regarding embryo-fetal toxicity for pregnant individuals. Other common but less severe adverse events reported by patients include nausea, fatigue, vomiting, diarrhea, decreased appetite, abdominal pain, hair loss, and fever.

Important: signs of low blood counts (fever, mouth sores, severe fatigue, easy bruising/bleeding — call your oncologist for fever ≥100.4°F/38°C). Severe diarrhea can lead to dehydration — stay hydrated; use loperamide for diarrhea after consulting your oncologist. CBC is monitored regularly.
No. Lonsurf is approved for patients with metastatic colorectal or gastric cancer who have already received standard prior therapies (fluoropyrimidine, oxaliplatin, irinotecan-based therapy, and biologic therapy with anti-VEGF and/or anti-EGFR for colorectal). It is not first-line therapy. Earlier-line treatment uses standard chemotherapy combinations with or without targeted agents and immunotherapy.
