Kymriah (tisagenlecleucel) is a one-time CAR-T cell therapy — your own T cells are collected, genetically engineered to express a chimeric antigen receptor (CAR) targeting CD19, expanded in a manufacturing facility, then infused back into you to attack CD19-expressing B-cell malignancies. It produces durable remissions in many patients with otherwise refractory disease.
How Kymriah Works:
CAR-T cell therapy is an advanced treatment that reprograms a patient’s own immune cells to selectively target and destroy B-cell cancer cells. The process involves collecting the patient’s T cells and genetically engineering them to express a chimeric antigen receptor that targets the CD19 protein. Once expanded and re-infused following chemotherapy, these modified cells multiply significantly within the body to attack the cancer. This targeted approach has helped many patients achieve long-lasting complete remission, though it can also cause serious toxicities.
Form and use:
Kymriah is administered as a single, one-time intravenous infusion delivered exclusively at a certified CAR-T treatment center. Prior to receiving the infusion, patients undergo three to five days of lymphodepleting chemotherapy, with the entire timeline from cell collection to infusion taking three to six weeks. Following the infusion, patients are hospitalized for a minimum of seven to ten days so providers can closely monitor them for severe complications. To ensure patient safety, the medication tocilizumab must be kept on standby to immediately manage potential cytokine release syndrome.
Generic availability:
There is currently no biosimilar version of Kymriah available on the market. Depending on the patient’s specific age, medical history, and institutional capabilities, healthcare providers may consider alternative CD19-targeted CAR-T therapies like Yescarta, Breyanzi, or Tecartus. Yescarta and Breyanzi serve as options for large B-cell and follicular lymphomas, while Tecartus is utilized specifically for adult patients with relapsed or refractory B-cell acute lymphoblastic leukemia.
Warnings:
Kymriah carries prominent boxed warnings for life-threatening risks, specifically cytokine release syndrome and severe neurologic toxicities like ICANS. The treatment also places patients at high risk for serious infections due to prolonged low blood cell counts and long-term depletion of healthy B cells, which often requires ongoing antibody replacement therapy. Furthermore, it carries risks for tumor lysis syndrome, allergic reactions, and secondary T-cell malignancies. Because of these severe safety concerns, the therapy is strictly restricted to certified centers operating under a mandatory REMS safety program.

Your own T cells are collected, genetically engineered to express a receptor that recognizes a cancer marker (CD19 for B-cell malignancies), expanded over weeks, then infused back into you. The modified T cells then expand massively in your body and kill the cancer cells. It is a one-time treatment that can produce durable remissions in patients with otherwise refractory disease.
FDA-approved indications include: pediatric/young adult (≤25 years) relapsed/refractory B-cell ALL, adult relapsed/refractory DLBCL after ≥2 lines of therapy, adult relapsed/refractory follicular lymphoma after ≥2 lines of therapy. Eligibility within these indications also depends on performance status, organ function, prior therapies, and absence of certain comorbidities. Treatment is at certified CAR-T centers.
Cytokine release syndrome (CRS): a systemic inflammatory response from massive CAR-T cell expansion and cytokine release — fever, hypotension, hypoxia, organ dysfunction. Managed with tocilizumab (anti-IL-6R) and supportive ICU-level care. ICANS (immune effector cell-associated neurotoxicity syndrome): neurologic dysfunction — confusion, seizures, encephalopathy. Both typically peak 1–14 days after CAR-T infusion. Both are managed in specialized centers with experienced teams.
In 2024, the FDA added a boxed warning to all CD19 CAR-T therapies (Kymriah, Yescarta, Breyanzi, Tecartus, etc.) for risk of T-cell malignancies — including some cases where the malignancy contained the CAR transgene, suggesting causation. The absolute risk appears low (<1%) compared to the substantial benefit in refractory disease, but lifelong monitoring is recommended.
Medicare covers CAR-T therapy with national coverage determination — but coverage details and out-of-pocket costs vary. Medicare Part A covers the inpatient hospitalization; the drug component depends on Part B or specific NCD provisions. The manufacturer’s PAP may help with copays or other costs. Coordination with the treatment center’s financial team is essential.
