Jynarque (tolvaptan) is a twice-daily oral selective vasopressin V2 receptor antagonist used to slow kidney function decline in ADPKD. It is the first FDA-approved therapy specifically to slow ADPKD progression — a major therapeutic advance for a disease that previously had only supportive care.
How Jynarque Works:
Autosomal dominant polycystic kidney disease (ADPKD) causes fluid-filled cysts to enlarge and replace normal kidney tissue, leading to progressive loss of kidney function. Vasopressin stimulates the production of cAMP through V2 receptors on kidney collecting ducts, which drives this cyst growth. Tolvaptan selectively blocks these V2 receptors, successfully reducing cAMP production and slowing both cyst growth and kidney function decline. Clinical trials show it slows the decline of estimated glomerular filtration rate (eGFR), though the trade-off includes a risk of hepatotoxicity and significantly increased urine output.
Form and use:
Jynarque is taken orally twice daily, with a larger dose administered in the morning and a smaller dose in the afternoon. Treatment starts at 45 mg in the morning and 15 mg in the afternoon, with a weekly titration up to a target dose of 90 mg and 30 mg. It is taken with food, and patients must drink water freely to manage the resulting aquaresis. Because of safety risks, it is available only through a restricted REMS program, and liver function tests (LFTs) are required at baseline, monthly for the first 18 months, and every 3 months thereafter.
Generic availability:
As of 2026, there is no generic version of Jynarque available in the United States. While generic tolvaptan tablets exist under the name Samsca, they are indicated for hyponatremia and are neither dosed nor FDA-approved for ADPKD. Consequently, Jynarque remains the only FDA-approved disease-modifying therapy specifically available for ADPKD patients. Management also typically relies on important adjunctive measures, such as blood pressure control with ACE inhibitors or ARBs, sodium restriction, and increased water intake.
Warnings:
Jynarque carries a boxed warning for serious and potentially fatal liver injury, necessitating a strict schedule of liver function tests at baseline, every 2 weeks for the first 18 months, monthly to month 30, and every 3 months thereafter. Due to these hepatic risks, the medication is strictly limited to a REMS distribution program. Additional warnings include dehydration, hypernatremia, and significant drug interactions with CYP3A inhibitors and inducers. Furthermore, the resulting aquaresis causes large urine volumes that can interfere with a patient’s sleep and daily activities.

Jynarque slows the rate of kidney function decline (eGFR loss) in ADPKD patients at risk of rapid progression. Clinical trials showed that over 3 years, the rate of eGFR decline was about 1 mL/min/1.73 m² per year slower on tolvaptan than placebo — meaningful over decades of ADPKD progression. Jynarque does not reverse existing cysts or restore lost kidney function; it slows further decline.
Tolvaptan can cause serious, potentially fatal liver injury (the boxed warning). Most LFT abnormalities are detected by the rigorous monitoring schedule: baseline, every 2 weeks for the first 18 months, then monthly to month 30, then every 3 months. Tolvaptan causes ‘aquaresis’ — selective excretion of free water without sodium. You’ll produce large volumes of dilute urine (sometimes 5–10+ liters per day). To prevent dehydration and hypernatremia, drink water freely whenever thirsty. This is a major lifestyle effect — affecting sleep (frequent nighttime urination) and daily life. Plan ahead for water access and bathroom access.
Jynarque is for ADPKD patients at risk of rapid progression — typically those with confirmed ADPKD diagnosis, eGFR appropriate for the indication, and clinical/imaging evidence of rapid kidney enlargement or eGFR decline. Your nephrologist will determine if you meet criteria. Patients with very advanced kidney disease may not benefit. Patients without rapid progression may not have a favorable risk/benefit balance.
