Invega Sustenna is a monthly IM long-acting injectable paliperidone — the foundation of the Invega LAI family. It delivers stable paliperidone levels over 30 days, addressing the adherence challenges of daily oral antipsychotics.
How It Works:
Adherence to daily oral antipsychotic medications is a major challenge in schizophrenia — nonadherence drives relapse, hospitalization, and poor long-term outcomes. Long-acting injectable (LAI) antipsychotics ensure therapeutic drug levels for weeks to months, dramatically improving adherence. Paliperidone (the active metabolite of risperidone) blocks dopamine D2 and serotonin 5-HT2A receptors — an atypical antipsychotic. Paliperidone palmitate is the palmitate ester injected IM as a depot — sustained release over 30 days for Invega Sustenna. It requires initial loading doses (day 1 and day 8) followed by monthly maintenance, and can be transitioned to Invega Trinza (3-monthly) or Invega Hafyera (6-monthly) once stable.
Form and Use:
Invega Sustenna IM injection in the deltoid or gluteal muscle. Loading dose: 234 mg IM on day 1 and 156 mg IM on day 8 (both deltoid), then 39–234 mg IM monthly maintenance based on response. Cold storage until use. Given at a psychiatric clinic or community mental health center.
Generic Availability:
No generic Invega Sustenna. Related paliperidone LAIs: Invega Trinza (3-monthly IM), Invega Hafyera (6-monthly IM) — same molecule, extended intervals after stabilization. Related LAIs (all brand): Abilify Maintena (aripiprazole monthly IM), Aristada (aripiprazole lauroxil monthly IM), Risperdal Consta (risperidone every 2 weeks IM), Zyprexa Relprevv (olanzapine every 2 or 4 weeks IM — post-injection sedation/delirium risk), Uzedy (risperidone monthly or every 2 months SC), Perseris (risperidone monthly SC). Oral antipsychotics: risperidone, olanzapine, quetiapine, aripiprazole, paliperidone, ziprasidone (mostly generic — very cheap). NAMI, Schizophrenia and Related Disorders Alliance. Community mental health centers often provide LAI programs.
Warnings:
Increased mortality in elderly patients with dementia-related psychosis (boxed warning — not approved for this), neuroleptic malignant syndrome, tardive dyskinesia, metabolic effects (weight gain, dyslipidemia, hyperglycemia — atypical class effect), hyperprolactinemia (menstrual irregularities, gynecomastia, sexual dysfunction), orthostatic hypotension. Other: injection site reactions, sedation, akathisia, EPS. The long-acting nature means side effects may persist for weeks after the last dose. Psychiatric monitoring is essential.
$1,600–$2,000 per monthly injection at specialty pharmacies or psychiatric clinics ($20,000+ annually). Generic oral paliperidone is ~$50/month; generic oral risperidone $10–$20/month. The LAI advantage is adherence, not cost per month.
Paliperidone is the active metabolite of risperidone — same class. Oral formulations require daily pill-taking; nonadherence is a major cause of schizophrenia relapse. Invega Sustenna monthly IM injection guarantees therapeutic drug levels for 30 days — same medication, dramatically different adherence profile.
All three are paliperidone palmitate depot injections at different intervals. Invega Sustenna is monthly (the foundation and starting LAI). Invega Trinza (3-monthly) requires prior stabilization on Sustenna for 4+ months. Invega Hafyera (6-monthly) requires prior stabilization on Sustenna for 4+ months or Trinza for 1 dose cycle. Progressive interval extension as the patient is stable.
Adherence — nonadherence to daily oral antipsychotics drives schizophrenia relapse, hospitalization, and poor outcomes. LAIs guarantee therapeutic drug levels for weeks. Growing evidence shows LAIs improve outcomes vs oral in appropriately selected patients — including many first-episode and early-course patients. Discuss with your psychiatrist.
Standard atypical antipsychotic effects: weight gain, metabolic effects (dyslipidemia, hyperglycemia), hyperprolactinemia (menstrual irregularities, gynecomastia, sexual dysfunction), sedation, akathisia, EPS. Injection site reactions are common. The long-acting formulation means side effects may persist for weeks after the last dose.
