Understanding Imovax:

Imovax (HDCV rabies vaccine) is used for pre-exposure prophylaxis (occupational risk — veterinarians, animal handlers, lab workers, travelers to endemic regions) and for post-exposure prophylaxis after suspected rabies exposure (animal bite, scratch, contact with bat). Untreated rabies is virtually 100% fatal — timely post-exposure prophylaxis is essential.

How Imovax Works:

Rabies virus is a neurotropic Lyssavirus transmitted by infected animal bites (dogs, cats, raccoons, skunks, bats, foxes). Once symptomatic, rabies is virtually always fatal — but post-exposure prophylaxis (PEP) initiated promptly is nearly 100% effective.

Imovax is an inactivated rabies vaccine grown in human diploid cells. Stimulates production of neutralizing antibodies against rabies virus glycoprotein. For PEP after exposure: 4 doses on days 0, 3, 7, 14 (with rabies immunoglobulin/RIG on day 0 for previously unvaccinated patients). For pre-exposure: 3 doses on days 0, 7, 21 or 28. Booster recommendations based on antibody titers.

Form and use:

Imovax given as IM injection in deltoid (or anterolateral thigh in infants). Do NOT give in gluteal area (reduced immunogenicity). Reconstituted from lyophilized vaccine just before administration.

Generic availability:

No generic. Alternative: RabAvert (PCECV) — comparable efficacy. Rabies immunoglobulin (RIG, HyperRAB, Imogam) given on day 0 of PEP for previously unvaccinated patients — infiltrated around wound and remainder IM at distal site.

Warnings:

Common reactions: local injection site reactions, headache, nausea, abdominal pain, muscle aches, dizziness. Rare serious: Guillain-Barré syndrome (very rare), severe allergic reactions, encephalopathy. Pregnant patients should receive PEP if exposed (rabies risk far exceeds vaccine risk).
