Gilenya (fingolimod) is a once-daily oral sphingosine-1-phosphate (S1P) receptor modulator used to treat relapsing forms of multiple sclerosis. By trapping autoreactive lymphocytes in lymph nodes, fingolimod reduces immune attack on the central nervous system.
How Gilenya Works:
Multiple sclerosis is driven by autoreactive T-cells and B-cells migrating into the central nervous system to attack myelin and axons. Fingolimod works by being phosphorylated into an active form that binds to sphingosine-1-phosphate (S1P) receptors on these lymphocytes, triggering receptor internalization. Without functional S1P receptors, lymphocytes are effectively “trapped” inside lymphoid tissues and cannot exit to migrate to the CNS. This mechanism causes a 70–80% drop in circulating lymphocyte counts within weeks, leading to a reduced relapse rate, decreased MRI lesion activity, and slowed disability progression.
Form and use:
Gilenya is administered orally as a once-daily capsule taken with or without food, with a standard adult dose of 0.5 mg and weight-based pediatric dosing for patients aged 10 and older. Due to cardiac risks, a first-dose observation (FDO) is strictly required, which involves 6 hours of continuous cardiac monitoring after the initial dose to check for bradycardia. Before initiating treatment, patients must undergo an extensive baseline screening process. This screening includes an electrocardiogram (ECG), an ophthalmologic exam, a varicella titer, a complete blood count (CBC), and liver function tests (LFTs).
Generic availability:
Generic fingolimod is widely available in the United States at a substantially lower cost than the brand-name drug Gilenya. Patients also have access to other specialized S1P modulators, such as siponimod (for active secondary progressive MS), ozanimod, and ponesimod. Furthermore, the broader landscape of multiple sclerosis disease-modifying therapies (DMTs) includes generic options like glatiramer acetate, dimethyl fumarate, and teriflunomide. A variety of other advanced treatments, ranging from interferons to monoclonal antibodies like natalizumab and ocrelizumab, are also available depending on the patient’s specific clinical needs.
Warnings:
Gilenya carries prominent warnings for bradyarrhythmia and atrioventricular (AV) block, necessitating strict first-dose cardiac observation. It also increases the risk of serious infections, including herpes, cryptococcal meningitis, and progressive multifocal leukoencephalopathy (PML), alongside risks of macular edema and hepatotoxicity. Patients may experience decreased respiratory function, elevated blood pressure, basal cell carcinoma, posterior reversible encephalopathy syndrome (PRES), or tumefactive MS lesions. Finally, the drug can cause fetal harm, requiring effective contraception during and for 2 months after treatment, and discontinuing it can trigger a severe rebound flare of MS disease activity.

Fingolimod can cause bradycardia (slowed heart rate) and rarely AV conduction block, especially with the first dose. Six hours of continuous cardiac monitoring after the first dose is required to detect any significant bradycardia or AV block. Observation includes baseline and post-dose ECG. The observation requirement applies to the first dose, doses restarted after >14 days off, or other specified scenarios.
Before starting fingolimod, you’ll need a recent ECG, ophthalmologic exam (to detect macular edema), varicella titer (consider varicella vaccination if seronegative — wait 4 weeks after live vaccine before starting), CBC, LFTs, and pregnancy test for those of childbearing potential. Periodic follow-up exams (eye, LFTs, BP, CBC) are needed during therapy.
Be very careful — severe MS rebound activity (worse than pre-treatment) has been reported after discontinuation, especially with prolonged interruption. If discontinuation is being considered, it should be done with close neurology supervision and often with transition to another DMT to bridge the lymphocyte recovery period.
Newer S1P modulators (siponimod, ozanimod, ponesimod) are more selective — primarily targeting S1P1 and S1P5 — which may translate to fewer cardiac effects, less need for first-dose observation, and shorter washout periods. Gilenya remains first-line for many patients but the newer agents may be preferred in selected situations.
If denied, we explore alternatives — switching to generic fingolimod (much cheaper), or to another MS DMT with its own PAP (oral options include generic dimethyl fumarate, generic teriflunomide; infusion/injection options include glatiramer, interferons, ocrelizumab, ofatumumab), the manufacturer’s copay program for commercially insured patients, or independent foundations such as the National MS Society, MS Association of America, PAN Foundation, or HealthWell Foundation.
