Epzicom (abacavir/lamivudine) is a once-daily fixed-dose combination NRTI backbone tablet used as part of combination antiretroviral therapy for HIV-1. It combines two NRTIs — abacavir (a guanosine analog) and lamivudine (a cytidine analog) — to provide a complete NRTI backbone in one tablet, which is then combined with a third antiretroviral (typically an INSTI like dolutegravir).
How Epzicom Injection Works:
The HIV-1 virus relies on a reverse transcriptase enzyme to convert its RNA genome into DNA so it can integrate into host chromosomes. Epzicom provides a robust backbone by combining abacavir (a guanosine analog) and lamivudine (a cytidine analog) to attack this enzyme from two different bases. Once inside cells, both drugs are phosphorylated and compete with natural components to incorporate into viral DNA, effectively terminating the growing chain because they lack a 3′-OH group. Unlike tenofovir-based alternatives, this combination avoids kidney and bone toxicity, though it does require specific genetic testing before use to prevent a severe hypersensitivity reaction.
Form and use:
Epzicom is prescribed as a single tablet taken by mouth once every day, and it can be administered with or without food. To function effectively, it cannot be used alone and must be combined with at least one other antiretroviral medication, which is typically an INSTI or an NNRTI. Crucially, patients must undergo HLA-B5701 testing before starting the medication to screen for risks. Under no circumstances should Epzicom ever be given to a patient who tests positive for the HLA-B5701 allele.
Generic availability:
The generic version of abacavir/lamivudine is widely available across the U.S. at a significantly lower cost compared to the brand-name Epzicom. Many adults currently take this specific drug combination as a component of the single-tablet regimen known as Triumeq. The other primary backbone options available to patients are tenofovir/emtricitabine, sold as generic Truvada or brand-name Descovy. Choosing between these available backbones depends entirely on a patient’s HLA-B*5701 status, kidney function, bone health, and hepatitis B status.
Warnings:
Epzicom features a boxed warning for severe, sometimes fatal abacavir hypersensitivity reactions in HLA-B*5701-positive individuals, and the drug must never be restarted if a reaction is suspected. It also carries a boxed warning for severe, acute HBV exacerbations if lamivudine is stopped in patients who are co-infected with hepatitis B. Additional warnings include risks of lactic acidosis, severe hepatomegaly with steatosis, immune reconstitution syndrome, and hepatotoxicity, particularly in those with HBV or HCV co-infections. Finally, clinical literature notes a debated but possible increased risk of cardiovascular events associated with the abacavir component.

Standalone Epzicom or generic abacavir/lamivudine is useful when the third ARV needs to be different from dolutegravir (e.g., raltegravir, doravirine, certain NNRTIs) or in pediatric patients.

Tell your prescriber. Lamivudine has activity against HBV but lamivudine monotherapy for HBV can lead to HBV resistance. HIV/HBV co-infected patients typically need a tenofovir-containing regimen (better HBV coverage) rather than abacavir/lamivudine. Discuss with your HIV provider.
