Epivir (lamivudine, 3TC) is a once- or twice-daily nucleoside reverse transcriptase inhibitor (NRTI) used as a component of combination antiretroviral therapy for HIV-1. It is closely related to emtricitabine (FTC) and is also active against hepatitis B (at a higher dose, marketed as Epivir-HBV).
How Emend Injection Works:
HIV is a retrovirus — it uses reverse transcriptase to convert its RNA genome into DNA. NRTIs are nucleoside analogs that get incorporated into the growing viral DNA chain by reverse transcriptase, but lack the 3′-OH needed to extend the chain — terminating elongation.
Lamivudine is a cytidine analog. Inside cells, it is phosphorylated to its active triphosphate form, which competes with natural dCTP for incorporation into viral DNA. Once incorporated, it terminates the chain. Lamivudine has very low toxicity, well-established efficacy, and forms part of the NRTI backbone in many modern HIV regimens (often combined with abacavir or tenofovir).
Form and use:
Epivir is taken as 300 mg once daily or 150 mg twice daily by mouth, with or without food. Oral solution available for pediatric patients and those who cannot swallow tablets. Combined with at least one other antiretroviral.
Generic availability:
Generic lamivudine is widely available in the U.S. at substantially lower cost than brand Epivir. Most adults take lamivudine as part of a combination tablet (Dovato/dolutegravir/lamivudine, Triumeq/dolutegravir/abacavir/lamivudine, Combivir/zidovudine/lamivudine generic, Trizivir, Epzicom). The 150 mg dose (Epivir-HBV) is used for chronic hepatitis B in HIV-negative patients.
Warnings:
Epivir carries a boxed warning for severe acute HBV exacerbation if discontinued in patients with HBV co-infection — HBV testing required before starting. Other warnings include lactic acidosis/severe hepatomegaly with steatosis (rare class effect), immune reconstitution syndrome, and risk of HIV resistance if used as functional monotherapy.

Tell your prescriber. Lamivudine is active against HBV — but lamivudine monotherapy for HBV in HIV co-infection can lead to HBV resistance. Patients with HIV/HBV co-infection typically need a regimen with two HBV-active drugs (lamivudine + tenofovir). Discuss with your HIV provider.
That’s a clinical decision between you and your HIV provider. Combination single-tablet regimens (Biktarvy, Triumeq, Dovato) simplify dosing and improve adherence. Standalone Epivir is less common except for specific clinical scenarios.
