Eosinophilic esophagitis is a chronic Th2-driven immune-mediated esophageal disease — eosinophil infiltration causes esophageal dysfunction, dysphagia, food impaction, and esophageal remodeling/strictures over time. Swallowed topical corticosteroid is foundational EoE therapy.

How Eohilia Works:

EoE involves Th2-driven eosinophil recruitment to esophagus, causing inflammation, remodeling, dysfunction. Swallowed topical steroid achieves high local mucosal concentration with limited systemic absorption.

Budesonide has high first-pass hepatic metabolism (~90%) — providing potent local anti-inflammatory effect with minimal systemic exposure. Viscous suspension formulation enhances esophageal contact time vs. swallowed inhaler formulations. Improves dysphagia, reduces eosinophil counts on biopsy, may reduce strictures with long-term use.

Form and use:

Eohilia 2 mg oral suspension taken twice daily (morning and evening) for 12 weeks. Swallow without water; don’t eat or drink for 30 minutes after to maximize esophageal contact. Rinse mouth with water after dose (reduces oral candidiasis risk).

Generic availability:

No generic Eohilia. Compounded budesonide oral viscous suspension (budesonide nebule 0.5 mg/2 mL mixed with sucralose syrup or honey) widely used pre-Eohilia approval — off-label but evidence-based and dramatically cheaper. Fluticasone metered-dose inhaler swallowed (with closed mouth, no spacer) — older approach, off-label, cheap.
