Elitek (rasburicase) is a recombinant urate oxidase enzyme used to rapidly lower plasma uric acid in patients at high risk for tumor lysis syndrome (TLS) — a potentially life-threatening complication of cancer treatment where rapid tumor cell death releases massive amounts of intracellular contents (uric acid, potassium, phosphate, calcium) into the bloodstream.
How Elitek Works:
Tumor lysis syndrome (TLS) is a metabolic emergency where massive cancer cell death releases intracellular contents like purines, phosphorus, and potassium into the blood. Purines metabolize into uric acid, which can crystallize in renal tubules and cause acute kidney injury, alongside dangerous electrolyte imbalances like hyperkalemia and hypocalcemia. Because humans lack functional urate oxidase to break down uric acid, rasburicase is used as a recombinant enzyme to quickly convert uric acid into a highly soluble substance called allantoin. This rapid reduction in uric acid burden helps clear the kidneys and prevents severe complications like acute renal failure, cardiac arrhythmias, and seizures.
Form and use:
Elitek is administered as an intravenous (IV) infusion over 30 minutes, given once daily for up to 5 days at a weight-based dose of 0.2 mg/kg/day. Healthcare providers frequently utilize a single-dose strategy, giving one weight-based dose and only administering additional doses if follow-up lab results show they are necessary. The medication comes in 3 mg or 7.5 mg vials that must be reconstituted before patient use. Because of severe drug interactions, G6PD testing should ideally be performed on the patient before starting this treatment whenever feasible.
Generic availability:
As of 2026, there is no biosimilar version of Elitek available in the United States. For lower-risk patients needing TLS prophylaxis, generic oral allopurinol is a much cheaper alternative, though it has a slower onset and is less effective for rapid urate reduction. Generic febuxostat is another oral option that serves as an alternative xanthine oxidase inhibitor for patients who cannot tolerate allopurinol. Essential supportive measures for TLS management include IV hydration and close electrolyte management, while pegloticase—another recombinant uricase—is reserved strictly for chronic gout and is not used for acute TLS.
Warnings:
Elitek carries severe boxed warnings for anaphylaxis, life-threatening methemoglobinemia, and acute hemolysis in patients with G6PD deficiency. Because rasburicase oxidizes hemoglobin in G6PD-deficient red blood cells, the drug is strictly contraindicated in these individuals, making pre-screening especially vital for patients of African, Mediterranean, or Southeast Asian descent. Additionally, the medication interferes with laboratory uric acid measurements because the drug will continue converting uric acid to allantoin inside the blood collection tube. To prevent falsely low lab results, blood samples must be placed on ice immediately and tested promptly by the laboratory.

Rasburicase generates hydrogen peroxide as a byproduct of converting uric acid to allantoin. G6PD-deficient red blood cells can’t detoxify hydrogen peroxide adequately — leading to oxidative damage and severe acute hemolysis. G6PD deficiency is more common in patients of African, Mediterranean, and Southeast Asian descent. G6PD testing before rasburicase is recommended when feasible — though urgent TLS situations may not allow time.
Allopurinol (oral, generic) blocks new uric acid formation by inhibiting xanthine oxidase — but does NOT lower existing uric acid. Takes days to weeks for full effect. Used for TLS prophylaxis. Elitek (rasburicase, IV) actively breaks down existing uric acid via urate oxidase — dramatically lowers uric acid within hours. Used for high-risk TLS prevention or established severe hyperuricemia. Both have a role; rasburicase is used when rapid uric acid reduction is critical.
