Cortifoam is hydrocortisone acetate 10% rectal foam — delivers topical corticosteroid to rectum and sigmoid colon for distal ulcerative colitis. Foam formulation provides good mucosal coverage and is often preferred over enemas by patients for ease of administration.

How Cortifoam Works:

Distal UC (proctitis, proctosigmoiditis) involves rectal and distal colonic mucosal inflammation. Topical corticosteroid achieves high local mucosal concentration with limited systemic absorption.

Hydrocortisone acetate is moderately potent corticosteroid. Foam formulation delivered rectally coats distal colonic mucosa. Reduces inflammation through standard corticosteroid mechanisms (inhibits phospholipase A2, reduces inflammatory mediator synthesis). Less systemic absorption than oral steroids — but more than budesonide foam (Uceris foam has higher first-pass metabolism).

Form and use:

Briviact is taken by mouth twice daily, with or without food. Tablets, oral solution, and IV form available. Starting dose 50 mg BID; max 100 mg BID. Renal/hepatic dose adjustment may be needed.

Generic availability:

Cortifoam: 1 applicator once or twice daily for 2–3 weeks, then every other day. Best at bedtime after bowel movement; retain as long as possible.

Warnings:

Topical steroid systemic effects with prolonged use or large doses: HPA axis suppression, cushingoid features, hyperglycemia, osteoporosis (long-term). Local: anorectal irritation, perforation (rare with prolonged use), masking of infection. Avoid in active untreated rectal infections.
