It is an artemisinin-based combination therapy (ACT) — the WHO-recommended global first-line treatment for falciparum malaria. The combination of fast-acting artemether and longer-acting lumefantrine provides rapid parasite killing with reduced resistance risk.
How Coartem Works:
Artemether/lumefantrine is a combination therapy designed to rapidly clear Plasmodium falciparum, the deadliest malaria parasite. Artemether is quickly activated to generate reactive radicals that damage multiple parasite targets for swift clearance, though it has a short half-life of about 2 hours. Lumefantrine has a much longer half-life of 3 to 6 days, providing sustained suppression to kill any residual parasites. Together, this combination ensures rapid cure while reducing the risk of drug resistance.
Form and use:
Coartem is administered as a 3-day course, requiring adults and children weighing 35 kg or more to take 4 tablets twice daily, totaling 24 tablets. Smaller children receive weight-based dosing instead of the standard adult regimen. Crucially, the medication must be taken with food or a fatty drink like milk. This is because lumefantrine absorption is highly dependent on food, which can increase drug exposure up to 16-fold compared to fasting.
Generic availability:
Generic versions of artemether/lumefantrine are widely available and significantly cheaper than the brand-name Coartem. While other combinations like artesunate/amodiaquine are used internationally, they are generally not FDA-approved or widely available in the US. For severe malaria cases involving high parasitemia or organ dysfunction, IV artesunate is the recommended first-line treatment. Warnings:
