Carbatrol is the extended-release formulation of carbamazepine — a sodium channel-blocking antiepileptic also used for trigeminal neuralgia and (as Equetro) bipolar disorder. Carbamazepine is one of the oldest and most widely used AEDs.

How Carbatrol Works:

Carbamazepine blocks voltage-gated sodium channels in neurons — reducing repetitive firing and stabilizing membranes. Effective for partial-onset and generalized tonic-clonic seizures, trigeminal neuralgia pain, and (in some patients) bipolar disorder.

ER formulation maintains 24-hour drug levels with twice-daily dosing, smoothing peak/trough variation and improving tolerability.

Form and use:

Carbatrol twice daily by mouth. Can be swallowed whole or opened and sprinkled on soft food. Start at low dose, increase weekly. Renal/hepatic dose adjustment may be needed.

Generic availability:

Generic carbamazepine ER and IR widely available. Tegretol/Tegretol XR are other brands. Equetro is for bipolar.

Warnings:

Carbamazepine carries BOXED WARNINGS for serious dermatologic reactions (Stevens-Johnson syndrome, TEN — HLA-B*1502 testing mandatory in Asian patients before starting due to ~10x higher risk; HLA-A*3101 also increases risk), aplastic anemia and agranulocytosis (rare but potentially fatal — CBC monitoring). Other warnings: hyponatremia/SIADH, hepatotoxicity, suicidal ideation, drug interactions (potent CYP3A4 inducer — reduces levels of many co-administered drugs including hormonal contraceptives, warfarin, many others), teratogenicity (neural tube defects, craniofacial defects — counsel female patients), withdrawal seizures.
