Cabenuva is the first long-acting injectable HIV-1 treatment — cabotegravir (INSTI) + rilpivirine (NNRTI) IM every 1 or 2 months. A complete HIV regimen for virologically suppressed patients that replaces daily oral therapy, addressing the adherence and stigma challenges of daily pill-taking.
How It Works:
HIV-1 treatment traditionally requires daily oral antiretroviral therapy. Adherence to daily pills can be challenging. Long-acting injectable HIV treatment reduces medical contact to monthly or every-2-month visits and eliminates daily pill-taking — for patients already virologically suppressed and appropriate for switch. Cabenuva combines cabotegravir (INSTI — blocking HIV integrase) and rilpivirine (NNRTI — blocking HIV reverse transcriptase). Both are long-acting depot IM injections. Every 1 month or every 2 months. Requires virologic suppression on a stable regimen before switch. ATLAS and FLAIR trials demonstrated non-inferiority to daily oral regimens for maintaining viral suppression.
Form and Use:
Cabenuva IM injection in the gluteus. Monthly regimen: cabotegravir 400 mg + rilpivirine 600 mg IM monthly. Every-2-month regimen: cabotegravir 600 mg + rilpivirine 900 mg IM every 2 months. Requires virologic suppression on stable ART before switch. May include an optional oral cabotegravir + rilpivirine lead-in for 4 weeks.
Generic Availability:
No generic Cabenuva. Other HIV treatment regimens (all daily oral): Biktarvy (bictegravir/emtricitabine/tenofovir alafenamide — most-prescribed single-tablet regimen), Dovato (dolutegravir/lamivudine — 2-drug single tablet), Triumeq, Genvoya, Odefsey, Symtuza. Generic oral HIV single-tablet regimens: Atripla generic, Complera generic, Symfi. Cabotegravir is also used as PrEP (Apretude every 2 months IM). Warnings:
Hypersensitivity reactions (including DRESS — rare but serious; contraindicated with history of hypersensitivity to cabotegravir or rilpivirine), hepatotoxicity (monitor LFTs), depression and suicidality (monitor for mood changes — INSTI class; screen before starting), injection site reactions (very common — pain, swelling, nodules), long pharmacological tail (many months after last injection — implications for missed doses and resistance risk), drug interactions (rifamycins contraindicated; other CYP3A4 inducers reduce levels), embryo-fetal considerations. Other: pyrexia, fatigue, headache, musculoskeletal pain, nausea, rash. Discontinuation planning is important given the long pharmacological tail.
