Kerendia is non-steroidal MRA — selective mineralocorticoid receptor blockade with less steroidal off-target effects than spironolactone (no gynecomastia, less hyperkalemia compared with spironolactone in trials).
How Kerendia Works:
Mineralocorticoid receptor activation drives renal/cardiac fibrosis and inflammation. MRA blockade slows disease progression.
FIDELIO and FIGARO trials: finerenone reduced composite CV and renal outcomes in T2DM CKD. Different evidence base from steroidal MRAs (spironolactone, eplerenone). Often used with SGLT2 inhibitor — together they reduce CKD progression.
Form and use:
Kerendia once daily. Dose 10 or 20 mg based on eGFR and serum K+.
Generic availability:
No generic. Steroidal MRAs (generic spironolactone, eplerenone) — different drug class. Aldactone for hyperaldosteronism/HF; eplerenone for HF post-MI. SGLT2 inhibitors (Jardiance, Farxiga) — different mechanism but overlapping CKD/CV benefits in T2DM.
Warnings:
Hyperkalemia (monitor K+ — initiation and dose adjustments), hypotension, hyponatremia (rare), drug interactions (strong CYP3A4 inhibitors — contraindicated; moderate — dose adjustment).
