Belsomra is dual orexin receptor antagonist (DORA) — blocks wake-promoting orexin system. Different mechanism from GABA-acting Z-drugs/benzodiazepines.
How Belsomra Works:
Orexin neuropeptides promote wakefulness. DORAs block orexin receptors — reduces wake signaling, promotes sleep.
Less abuse potential than Z-drugs, less withdrawal, no respiratory depression at therapeutic doses. Still controlled (Schedule IV).
Form and use:
Belsomra 10–20 mg within 30 min of bedtime, ≥7 hours before waking. Take on empty stomach.
Generic availability:
No DORA generic. Other DORAs: Quviviq (daridorexant), Dayvigo (lemborexant). Generic Z-drugs cheaper: zolpidem (Ambien), eszopiclone (Lunesta), zaleplon (Sonata). Generic ramelteon (Rozerem — melatonin agonist, non-controlled). Generic doxepin low-dose (Silenor). OTC melatonin.
Warnings:
CNS depression, complex sleep behaviors (sleep-eating, sleep-driving — rare), depression worsening, sleep paralysis, hallucinations, daytime sleepiness, narcolepsy-like symptoms, suicidality.
Belsomra $400–$700. Generic zolpidem $10–$30. OTC melatonin $10.
Less abuse potential, less withdrawal, less daytime sleepiness, no respiratory depression at therapeutic doses. But expensive vs generic Z-drugs.
