Zirgan is ophthalmic ganciclovir gel for HSV keratitis. Less frequent dosing (5x daily) and less corneal toxicity than older trifluridine. Topical antiviral therapy directly to infected cornea.

How Zirgan Works:

HSV keratitis (herpes simplex keratitis) — viral infection of cornea causing dendritic or geographic ulcers. Without treatment, can cause scarring, perforation, vision loss.

Ganciclovir is acyclic nucleoside analog — phosphorylated by viral thymidine kinase, inhibits HSV DNA polymerase. Topical gel formulation effective for corneal HSV with less toxicity than trifluridine.

Form and use:

Zirgan 1 drop affected eye 5 times daily until corneal ulcer heals, then 3 times daily for 7 days.

Generic availability:

No generic Zirgan currently. Trifluridine 1% (Viroptic) generic available — older alternative, 9 times daily dosing, more corneal toxicity. Oral acyclovir/valacyclovir (generic, very cheap) often used adjunct or alternative for HSV keratitis — increasing evidence for oral therapy comparable to topical.

Warnings:

Corneal toxicity (less than trifluridine), blurred vision after application, eye irritation. Concomitant topical steroid only with careful ophthalmologist guidance (steroids can worsen HSV).
