Understanding Wainua:

Wainua is an antisense oligonucleotide (ASO) that reduces transthyretin (TTR) production. Hereditary ATTR amyloidosis (hATTR) is a rare autosomal dominant disorder caused by mutations in TTR — abnormal TTR forms amyloid deposits in peripheral nerves (causing polyneuropathy), heart (cardiomyopathy), and other tissues. Reducing TTR production reduces amyloid burden and improves outcomes.

How Wainua Works:

TTR is produced primarily by the liver and circulates in blood as a tetramer. Mutated TTR in hATTR is unstable, dissociates into monomers, misfolds, and deposits as amyloid in peripheral nerves and/or heart. Reducing TTR production directly reduces substrate for amyloid formation.

Eplontersen is a GalNAc-conjugated antisense oligonucleotide that targets the liver via the asialoglycoprotein receptor. It binds TTR mRNA, triggering its degradation — reducing TTR production by approximately 80%. The GalNAc conjugation allows monthly SC self-injection (vs. weekly inotersen and IV infusions for patisiran). Outcomes: improved neuropathy progression, quality of life in hATTR-PN.

Form and use:

Wainua given as monthly subcutaneous self-injection (after training) — autoinjector device. Vitamin A supplementation 2,500–10,000 IU/day required (TTR carries vitamin A; reducing TTR reduces vitamin A binding/transport).

Generic availability:

No biosimilar. ATTR-PN alternatives: Tegsedi (inotersen — older ASO, weekly SC, more side effects), Onpattro (patisiran — siRNA, IV q3w), Amvuttra (vutrisiran — siRNA, SC q3m, also self-injected). For ATTR cardiomyopathy: tafamidis (Vyndaqel/Vyndamax), acoramidis (Attruby — newer 2024), Onpattro/Amvuttra also studied in ATTR-CM. All extraordinarily expensive.

Warnings:

Wainua warnings include vitamin A deficiency (mandatory vitamin A supplementation), injection site reactions, hepatotoxicity (LFT monitoring), thrombocytopenia (CBC monitoring — less severe than older inotersen).
