How Votrient Works:
Pazopanib is a medication that works by blocking multiple tyrosine kinases, including VEGFR, PDGFR, c-Kit, and FGFR. Cancers often rely on these specific signals to stimulate angiogenesis, which is the growth of new blood vessels needed to feed and expand tumors. By simultaneously halting these pathways, the drug effectively inhibits both blood vessel growth and direct tumor cell proliferation. This combined action ultimately results in tumor shrinkage and prolongs progression-free survival for patients.
Form and use:
This medication is supplied under the brand name Votrient and is taken orally once daily at a standard dose of 800 mg. It must strictly be taken on an empty stomach, meaning either at least 1 hour before a meal or 2 hours after one. Adhering to the empty-stomach rule is critical because consuming food significantly increases the absorption of the drug. Additionally, patients must ingest the tablets whole without crushing, chewing, or splitting them.
Generic availability:
Generic versions of pazopanib are currently available in the United States, offering a substantially lower cost compared to the brand-name drug Votrient. For advanced renal cell carcinoma (RCC), patients have alternative options such as sunitinib, cabozantinib, axitinib, tivozanib, or various immunotherapy combinations. If being treated for soft tissue sarcoma, alternative therapeutic options include doxorubicin, gemcitabine plus docetaxel, eribulin, and trabectedin.
Warnings:
Votrient carries a serious Boxed Warning for severe and potentially fatal hepatotoxicity, requiring regular liver function tests before and during treatment. The medication is also associated with cardiovascular risks, including QT prolongation, heart failure from LVEF decline, severe hypertension, and thromboembolic or hemorrhagic events. Furthermore, patients face potential risks of gastrointestinal perforations, wound healing complications, lung disease, and embryo-fetal toxicity. Blood pressure and electrolytes must be actively monitored and managed to mitigate these serious adverse effects.

Pazopanib can cause severe, sometimes fatal hepatotoxicity (the boxed warning). LFTs should be checked at baseline, every 2 weeks for the first 2 months, then monthly for at least 4 months. Significant LFT elevations may require dose interruption, reduction, or discontinuation. Tell your oncologist promptly about jaundice, severe fatigue, dark urine, light stools, or abdominal pain.
VEGFR-targeted therapies including Votrient commonly cause hypertension. Blood pressure should be measured at baseline and frequently during therapy. Antihypertensive treatment (ACE inhibitors, ARBs, calcium channel blockers, etc.) is often needed and is usually compatible with Votrient. Severe or uncontrolled hypertension may require dose adjustment of Votrient.
Yes. Pazopanib is a CYP3A substrate and a CYP3A/2C8/2D6 inhibitor. Strong CYP3A inhibitors raise levels; strong CYP3A inducers lower levels. Acid-reducing drugs (PPIs, H2 blockers) can reduce pazopanib absorption — timing precautions needed. Other QT-prolonging drugs should be avoided where possible. Tell every prescriber and pharmacist about Votrient.
