Viracept (nelfinavir) is first-generation unboosted HIV protease inhibitor from late 1990s. Twice-daily dosing, multiple drug interactions, GI side effects (diarrhea), modest potency. No longer in HIV guidelines.
How Viracept Works:
HIV protease cleaves viral polyprotein into functional viral proteins. PI inhibitors block this — viral particles become non-infectious. Nelfinavir was unboosted PI (no ritonavir/cobicistat boosting).
Modern HIV care uses INSTI-based regimens — Biktarvy, Triumeq, Dovato. Higher resistance barriers, simpler dosing, better tolerability, fewer drug interactions than older PIs.
Form and use:
Viracept BID with food — outdated regimen.
Generic availability:
Generic nelfinavir may be available but rarely prescribed. Modern preferred: Biktarvy (BIC/FTC/TAF), Triumeq (DTG/ABC/3TC), Dovato (DTG/3TC). Single-tablet once-daily regimens.
Warnings:
Severe diarrhea (most common side effect — often dose-limiting), hepatotoxicity, hyperglycemia/diabetes, fat redistribution, hyperlipidemia, drug interactions (CYP3A — many).
$400–$700/month. Modern regimens preferred.
Yes — discuss with HIV provider. Modern regimens (Biktarvy, Triumeq, Dovato) much better.
Twice-daily dosing, frequent diarrhea, drug interactions, lower potency. INSTI-based regimens superior in all aspects.
Most common Viracept side effect. Often improves but can be dose-limiting and disabling.
