Viberzi is an oral mixed opioid receptor modulator for IBS-D. Mu-agonist/delta-antagonist/kappa-agonist activity in enteric nervous system reduces gut motility, secretion, and visceral pain — without significant CNS opioid effects (poor CNS penetration).
How Viberzi Works:
IBS-D involves disordered gut motility, secretion, and visceral hypersensitivity. Enteric opioid receptors regulate these functions.
Eluxadoline acts peripherally on enteric mu, delta, and kappa opioid receptors. Mu-agonism reduces motility (slows transit, reduces diarrhea); delta-antagonism may reduce constipation risk seen with pure mu-agonists; kappa-agonism reduces visceral pain. Minimal CNS penetration limits central opioid effects (no euphoria, addiction concerns). Effective for IBS-D abdominal pain and stool consistency.
Form and use:
Viberzi 100 mg twice daily by mouth, with food. Lower dose (75 mg BID) for patients who can’t tolerate higher dose or with certain contraindications. CONTRAINDICATED in patients without gallbladder (post-cholecystectomy) due to increased pancreatitis risk.
Generic availability:
No generic Viberzi. Other IBS-D therapies: rifaximin (Xifaxan generic available — antibiotic with role in IBS-D), alosetron (Lotronex generic — REMS-restricted to women with severe IBS-D), loperamide (generic, OTC, very cheap — diarrhea control only), bile acid sequestrants (cholestyramine, colesevelam — for bile acid diarrhea), TCAs (amitriptyline, nortriptyline generic), antispasmodics (dicyclomine, hyoscyamine generic), low-dose rifaximin for SIBO overlap.
Viberzi CONTRAINDICATED in: patients without gallbladder (severely increased pancreatitis risk — boxed warning), known/suspected biliary duct obstruction, alcoholism/alcohol abuse, history of pancreatitis, severe hepatic impairment, severe constipation. Other warnings: sphincter of Oddi spasm, pancreatitis (acute pancreatitis can occur in patients with intact gallbladder), CNS effects (somnolence, fatigue, dizziness).
