Vaxneuvance is a 15-valent pneumococcal conjugate vaccine (PCV15) — covers 15 pneumococcal serotypes. Pneumococcal disease (invasive infection like meningitis/bacteremia, plus pneumonia) is a major cause of morbidity and mortality, especially in young children, elderly, and immunocompromised patients.
How Vaxneuvance Works:
Streptococcus pneumoniae causes pneumonia, otitis media, meningitis, bacteremia. Polysaccharide capsule is major virulence factor; antibodies to capsular polysaccharides confer protection.
PCV15 contains polysaccharides from 15 serotypes conjugated to a carrier protein (CRM197) — conjugation produces T-cell-dependent immune response with strong memory (vs. unconjugated PPSV23). Replaces or supplements older PCV13 in many adult and pediatric immunization schedules.
Form and use:
Vaxneuvance given as IM injection. Pediatric routine series at 2, 4, 6, and 12–15 months. Adult dosing varies by indication and prior pneumococcal vaccination history.
Generic availability:
No ‘generic.’ Pneumococcal vaccine alternatives: Prevnar 20 (PCV20 — broader coverage, increasingly preferred), Prevnar 13 (PCV13, older), Pneumovax 23 (PPSV23, polysaccharide). Current ACIP recommendations favor PCV20 for many adults; PCV15 if PCV20 unavailable, often followed by PPSV23. Discuss with provider about optimal pneumococcal vaccination strategy.
Warnings:
Common: injection site reactions, fatigue, headache, muscle ache. Rare severe reactions. Pregnant women: limited safety data with PCV15 specifically; PCV20 or other vaccines may have more data.
$200–$280/dose. Medicare Part B $0 cost-share. ACA preventive often $0. Insurance commercial may vary.
Vaxneuvance (PCV15) covers 15 serotypes; Prevnar 20 (PCV20) covers 20 serotypes. Current ACIP recommendations generally prefer PCV20 if available for most adults. PCV15 acceptable; if used, typically followed by PPSV23 after 1+ year.
All children: routine series at 2/4/6/12–15 months. Adults: depends on age, prior vaccination, risk factors. Adults ≥65 generally recommended. Adults 19–64 with risk factors (chronic medical conditions, immunocompromise, CSF leak, cochlear implant, etc.) recommended.
Yes — Medicare Part B covers pneumococcal vaccines (PCV20, PCV15, PPSV23) with $0 cost-share for beneficiaries. No out-of-pocket cost.
Most commercial insurance covers pneumococcal vaccines per ACIP recommendations with no cost-share under ACA preventive coverage.
If you receive PCV15, ACIP recommends PPSV23 typically 1+ year later (8 weeks later for immunocompromised). If you receive PCV20, additional PPSV23 not routinely needed.
