Sylatron is pegylated interferon alfa-2b for adjuvant treatment of melanoma after surgical removal of regional lymph nodes. Pegylation extends the half-life of interferon alfa-2b, allowing weekly dosing instead of the daily or thrice-weekly dosing required for older interferon preparations.
How Sylatron Works:
Sylatron (pegylated interferon alfa-2b) is an adjuvant therapy used after surgical removal of melanoma to eliminate remaining micrometastatic disease and prevent recurrence. It works through immunomodulatory and antitumor effects, such as activating natural killer cells, improving immune recognition, and directly stopping tumor cell growth. The addition of pegylation extends the drug’s half-life, which allows for convenient weekly dosing. While it improves relapse-free survival, modern treatment has largely shifted to checkpoint immunotherapy due to Sylatron’s high toxicity.
Form and use:
Sylatron is administered as a weekly subcutaneous injection that patients or caregivers can self-inject at home after proper training. The standard regimen begins with an induction phase of 6 mcg/kg/week for 8 doses, followed by a maintenance phase of 3 mcg/kg/week for up to 5 years. Because of the therapy’s intensity, dose modifications are frequently required to manage side effects. These adjustments are typically triggered by fatigue, depression, cytopenias, or hepatotoxicity.
Generic availability:
As of 2026, there is no biosimilar version of Sylatron available in the United States. Furthermore, the drug is rarely prescribed for new patients today because it has been largely replaced by more effective options. Modern preferred therapies include checkpoint inhibitors like nivolumab (Opdivo) and pembrolizumab (Keytruda), or the combination of dabrafenib and trametinib for BRAF V600+ mutations. These newer alternatives are favored because they offer substantially better efficacy and much lower toxicity.
Warnings:
Sylatron carries severe boxed warnings for depression, suicidality, serious psychiatric conditions, ischemic disorders, and potentially fatal hepatotoxicity. Patients very frequently experience severe, dose-limiting flu-like symptoms, including fever, chills, fatigue, and muscle aches. Additional significant risks involve cytopenias like neutropenia and anemia, as well as the development of autoimmune disorders such as thyroid dysfunction. It can also cause pulmonary, ophthalmologic, gastrointestinal, and dermatologic complications.

Modern preferred adjuvant options: nivolumab (Opdivo) or pembrolizumab (Keytruda) — checkpoint inhibitors with substantially better efficacy and lesser toxicity than interferon; for BRAF V600+ patients, adjuvant dabrafenib + trametinib (Tafinlar + Mekinist) is another preferred option. Adjuvant interferon (Sylatron) is now rarely the first choice for new patients due to inferior efficacy and substantially worse tolerability.
Sylatron has substantial side effects: flu-like symptoms (fever, chills, fatigue, myalgia — affect most patients, often severe), depression/suicidality (boxed warning — psychiatric monitoring essential), cytopenias, thyroid dysfunction, hepatotoxicity, vision problems (retinopathy), GI symptoms (nausea, vomiting, diarrhea), skin reactions. Toxicity is often the limiting factor — many patients require dose reductions or discontinuation. Quality of life on interferon is significantly impacted.
If you’ve been on Sylatron and tolerating it, the decision depends on where you are in the 5-year course and your clinical situation. For patients still in the early adjuvant period and tolerating interferon poorly, switching to checkpoint inhibitor immunotherapy is often reasonable. For patients near the end of the planned 5-year course who have done well, completing may be reasonable. Discuss with your oncologist.
Yes. Medicare Part D beneficiaries can typically qualify, especially if you face specialty-tier copays. With newer adjuvant options preferred for most patients, Sylatron PAP requests are less common in 2026.
If denied, we explore alternatives — switching to checkpoint inhibitor immunotherapy (Opdivo, Keytruda) which is now standard adjuvant therapy with strong PAP programs, BRAF/MEK combination if BRAF-mutated, the manufacturer’s copay program for commercially insured patients, or independent foundations such as the Melanoma Research Foundation, PAN Foundation, or HealthWell Foundation. Pegylation is the chemical attachment of polyethylene glycol (PEG) to a drug molecule. PEG attachment increases molecular size, prolonging half-life and reducing clearance. For Sylatron, pegylation extends interferon alfa-2b half-life enough for once-weekly dosing instead of the daily or thrice-weekly dosing required for older non-pegylated interferon. Pegylation also tends to reduce some side effects and improve patient adherence.
