Stromectol (ivermectin) is an oral antiparasitic agent used to treat several human parasitic infections. It is highly effective for strongyloidiasis (intestinal threadworm) and onchocerciasis (river blindness), and is widely used off-label for scabies (especially crusted/Norwegian scabies in immunocompromised patients), pediculosis (lice), and other parasitic conditions.
How Stromectol Works:
Ivermectin targets glutamate-gated chloride channels found in parasites, which mammals lack in their central nervous system. By binding to these channels, the drug increases chloride influx, leading to paralysis and death of the parasite. This selective toxicity makes it safe for mammals while effectively treating infections like strongyloidiasis, onchocerciasis, and scabies. Depending on the condition, it either clears the infection in a single dose or suppresses it through repeated treatments.
Form and use:
Stromectol is available as 3 mg tablets taken by mouth on an empty stomach with water. Dosing is weight-based, typically requiring a single dose of 200 mcg/kg for strongyloidiasis or 150 mcg/kg for onchocerciasis. For onchocerciasis, this single dose is repeated every 6 to 12 months to manage the infection over time. When used off-label for scabies, a 200 mcg/kg dose is given orally and repeated 7 to 14 days later.
Generic availability:
Generic ivermectin is widely available in the U.S. and costs substantially less than the brand-name Stromectol. Other alternative treatments exist, such as generic albendazole for nematode infections and topical permethrin 5% cream as a first-line option for scabies. Additionally, topical ivermectin 0.5% lotion is available specifically for treating head lice.
Warnings:
The drug can trigger a Mazzotti reaction in onchocerciasis patients, causing an intense but temporary inflammatory response as the parasites die. It can also cause severe encephalopathy in patients with a high Loa loa coinfection, making screening crucial before treatment. High doses can lead to CNS depression, and adverse effects may occur if a patient’s blood-brain barrier is disrupted. Finally, ivermectin is not FDA-approved for COVID-19, as clinical trials show no benefit, and it should only be used for approved indications under medical supervision.
