Understanding Stimate:

Stimate is high-concentration desmopressin (DDAVP) nasal spray (1.5 mg/mL) used to stimulate endogenous release of factor VIII and von Willebrand factor from endothelial stores. Useful for mild hemophilia A and type I vWD bleeding management — avoiding factor product infusions in mild cases.

How Stimate Works:

Desmopressin is synthetic vasopressin analog with selective V2 receptor activity. V2 stimulation triggers release of factor VIII and von Willebrand factor (vWF) from endothelial Weibel-Palade bodies — temporarily increasing circulating FVIII and vWF (3–5x baseline) for several hours.

Effective for mild hemophilia A (baseline FVIII 5–40%) and type I vWD (most common, mild vWD type). Not effective for severe hemophilia A (no FVIII stores), type 3 vWD, or type 2B vWD (causes thrombocytopenia).

Form and use:

Stimate intranasal spray (1 spray = 150 mcg, both nostrils for adults — total 300 mcg). Onset ~30 min, peak ~1–2 hours. Tachyphylaxis with frequent use (FVIII/vWF stores deplete) — limit to every 48 hours or use only acutely.

Generic availability:

Brand Stimate only (supply variable). IV desmopressin (DDAVP — generic, cheap) used in hospital for hemostatic indications. Low-concentration DDAVP nasal sprays (generic 0.01% — for diabetes insipidus only) — NOT appropriate for hemostatic indications (too low concentration).

Warnings:

Hyponatremia (CRITICAL — fluid restriction during use; monitor serum sodium with repeated dosing; can cause seizures; deaths reported), water intoxication, thrombosis (rare), hypersensitivity reactions, tachyphylaxis with frequent use.
