Midostaurin works by inhibiting both wild-type and mutant FLT3, which significantly improves event-free and overall survival when added to standard induction and consolidation chemotherapy. Additionally, the drug targets KIT mutations, including the common KIT D816V driver mutation. This specialized KIT inhibition effectively reduces mast cell burden and improves overall disease control for patients with advanced systemic mastocytosis.

Form and use:

For newly diagnosed FLT3-positive AML, Rydapt is administered as a 50 mg oral dose taken twice daily with food on days 8 through 21 of each 28-day chemotherapy cycle. Following the induction and consolidation phases, it can be continued as a single-agent maintenance therapy for up to 12 cycles. For patients treating advanced systemic mastocytosis, the medication is prescribed at a higher dose of 100 mg twice daily taken continuously. Generic availability:

As of 2026, there is no generic version of Rydapt available in the United States. Other specialized FLT3 inhibitors on the market include gilteritinib, which is approved for relapsed or refractory FLT3-positive AML, and quizartinib, a newer therapy used alongside induction chemotherapy. For the treatment of systemic mastocytosis, avapritinib is another available option. Avapritinib is often preferred over midostaurin for many mastocytosis patients due to its superior selectivity for the KIT D816V mutation.

Warnings:
